空肠十二指肠侧侧垂直褥式缝合技术的临床应用

摘　　要：目的探讨空肠十二指肠侧侧垂直褥式缝合技术在结肠癌侵犯十二指肠消化道重建中的应用价值。方法回顾性分析海南省肿瘤医院2015年10月至2019年1月收治的9例结肠癌侵犯十二指肠患者的临床病理资料。所有患者行肿瘤根治术后,均采用空肠十二指肠侧侧垂直褥式缝合技术进行消化道重建,回肠横结肠行端侧或侧侧吻合。分析9例患者的手术时间、术中出血量、术后吻合口瘘及吻合口出血等并发症发生情况。结果所有患者均获得肿瘤R0切除,并按空肠十二指肠侧侧垂直褥式缝合技术完成消化道重建。手术时间(180±23)min,术中出血量(190±40)ml,9例患者无吻合口瘘及吻合口出血发生。术后排气时间为3~6 d。术后1例患者出现胃瘫,经保守治疗后好转出院。术后平均住院时间为13 d。结论空肠十二指肠侧侧垂直褥式缝合技术应用于结肠癌侵犯十二指肠的消化道重建,具有手术风险小、操作简单的优点,值得临床推广应用。Objective To evaluate the clinical value of jejunal duodenal lateral vertical pad suture technique in the reconstruction of digestive tract invaded by colon cancer.Methods The clinical data of 9 cases of colon cancer patients with duodenal invasion admitted to hainan cancer hospital from October 2015 to January 2019 were retrospectively analyzed.After radical resection of tumor,all patients underwent jejunal duodenal lateral vertical pad intestinal suture technique for digestive tract reconstruction,and lateral or lateral anastomosis was performed for transverse ileum.The operative time,intraoperative blood loss,postoperative anastomotic fistula and anastomotic bleeding of 9 patients were analyzed.Results R0 resection and gastrointestinal reconstruction were successfully performed in all patients.The operation time was(180±23)min,and the intraoperative blood loss was(190±40)ml.No anastomotic fistula or anastomotic bleeding occurred in 9 patients.Postoperative exhaust time was 3-6 days.After the operation,1 patient developed gastroparesis and was discharged after conservative treatment.The mean postoperative stay was 13 days.Conclusions Jejunal and duodenal lateral vertical mattress suture technique,which has the advantages of low surgical risk and simple operation in reconstruction of digestive tract of coloc cancer with duodenum invasion,is worthy of clinical application.
