摘  要:目的多层螺旋CT对胰腺乏血供神经内分泌肿瘤(hypo-PNET)与胰腺浆液性微囊腺瘤(SM-CA)的鉴别诊断价值。方法回顾性分析病理证实的14例hypo-PNET及12例SMCA患者的临床及影像资料,对比分析2组患者的年龄、性别及病变的部位、形态、大小、边界、密度、纤维间隔、强化峰值、有无胰管扩张、钙化、有无转移或侵犯。结果Hypo-PNET组与SMCA组患者年龄差异无统计学意义(P>0.05),2组性别差异具有统计学意义(P<0.05),SMCA多发于女性。Hypo-PNET组与SMCA组在病变边界、纤维间隔、强化峰值时相、转移或侵犯方面差异具有统计学意义(P<0.05)。SMCA组较Hypo-PNET组病变边界清楚、出现纤维间隔的概率高,Hypo-PNET组较SMCA组更容易出现强化峰值在动脉期、远处转移或侵犯。2组病例发病部位、形态、大小、密度、胰管扩张、钙化方面差异无统计学意义(P>0.05)。结论多层螺旋CT对hypo-PNET及SMCA具有鉴别诊断价值。Objective To investigate the value of the MSCT for differentiation ofhypovascular pancreatic neuroendocrine tumors(hypo-PNET)from pancreatic serous microcystic adenoma(SMCA).Methods The clinical and imaging data of 14 patients with hypo-PNET and 12 patients with SMCA confirmed by pathology were retrospectively analyzed.The clinical and CT data included the age,gender,and location,shape,size,boundary,density,fibrous interval,peak phase ofenhancement,pancreatic duct dilation,metastasis or invasion.Results There was significant difference in gender between the hypo-PNET group and the SMCA group(P<0.05).There were statistically significant differences between hypo-PNET group and SMCA group in terms of boundary,fibrous interval,peak phase ofenhancement,metastasis or invasion.Compared with hypo-PNET group,SMCA group had clearer lesion boundaries and higher probability of fibrous interval.Hypo-PNET group was more likely to have the peak enhancement occurred in arterial phase and metastasis or invasion than SMCA group.There was no statistically significant difference in the location,shape,size,density,pancreatic duct dilatation,and calcification between the two groups of patients(P>0.05).Conclusion Multi-slice spiral CT has differential diagnostic value for hypo-PNET and SMCA.
