水平肌加强减弱术联合下斜肌切断减弱术治疗斜视伴下斜肌功能亢进患儿疗效评价

摘　　要：目的探讨水平肌加强减弱术联合下斜肌切断减弱术治疗斜视伴下斜肌功能亢进(IOOA)患儿的疗效。方法选取我院收治的斜视伴IOOA患儿72例(2018年2月~2019年4月),按照手术方案不同分组,对照组(36例,55只眼)采用水平肌加强减弱术+下斜肌后徙术,观察组(36例,57只眼)采用水平肌加强减弱术+下斜肌切断减弱术。比较两组斜视改善情况、IOOA改善率、术前、术后1个月斜视角度变化(垂直斜视角、水平斜视角)。结果观察组斜视改善优良率97.22%(35/36)、IOOA改善率94.44%(34/36)较对照组77.78%(28/36)、75.00%(27/36)高(x1^2=4.571,P1=0.033;x2^2=5.258,P2=0.022);术后1个月观察组垂直斜视角、水平斜视角小于对照组(t1=32.145,P1=0.000;t2=23.497,P2=0.000)。结论水平肌加强减弱术+下斜肌切断减弱术对斜视伴IOOA患儿疗效确切,可调整斜视角度,矫正斜视症状,改善下斜肌功能,有助于恢复患儿正常视功能。Objetive To investigate the effect of horizontal muscle strengthening and weakening surgery+inferior oblique muscle cutting and weakening surgery on children with strabismus accompanied by inferior oblique muscle hyperfunction(IOOA).Methods Seventy-two children with strabismus accompanied by IOOA(February 2018 to April 2019)in our hospital were selected and grouped according to different surgical schemes.The control group(36 cases,55 eyes)was treated with horizontal muscle strengthening and weakening surgery+inferior oblique receding surgery,and the observation group(36 cases,57 eyes)was treated with horizontal muscle strengthening and weakening surgery+inferior oblique muscle cutting and weakening surgery.The improvement of strabismus,the improvement rate of IOOA,and the change of strabismus angle(vertical oblique angle and horizontal oblique angle)before and 1 month after surgery were compared between the two groups.Results The improvement rate of strabismus in the observation group was 97.22%(35/36),and the improvement rate of IOOA was 94.44%(34/36),which was higher than 77.78%(28/36 and 75.00%(27/36))of the control group(x 1^2=4.571,P,=0.033;x2^2=5.258,P2=0.022);at 1 month after operation,the vertical oblique angle and horizontal oblique angle of the observation group were smaller than the control group(t1=32.145,P1=0.000;t2=23.497,P2=0.000).Conclusion The horizontal muscle strengthening and weakening surgery+inferior oblique muscle cutting and weakening surgery is effective for children with strabismus accompanied by IOOA,the strabismus angle can be adjusted,the symptoms of strabismus can be corrected,and the function of the inferior oblique muscle can be improved,which can help restore normal visual function.
