机器人辅助腹腔镜前列腺癌根治术的效果评价

摘　　要：目的对比T2期以下患者普通腹腔镜与达芬奇机器人辅助腹腔镜前列腺癌根治术的临床治疗效果。方法回顾性研究2013年1月—2018年12月间我院收治的128例T2期以下前列腺癌患者,根据手术入路和手术方式的不同分为对照组(68例)和研究组(60例),对照组采用经腹膜外途径腹腔镜前列腺癌根治术(extraperitoneal laparoscopic radical prostatectomy,ELRP),研究组施行达芬奇机器人辅助腹腔镜前列腺癌根治术(robot-assisted laparoscopic radical prostatectomy,RALRP),比较两组的临床疗效。结果研究组患者的膀胱尿道吻合时间是(18.75±1.42)min,留置引流管时间是(5.14±0.47)d,术后住院时间是(5.27±0.48)d,均显著短于对照组(P<0.05);术中出血量是(94.27±9.05)mL,显著少于对照组(P<0.05);研究组患者的总手术时间是(156.86±11.05)min,肠功能恢复时间是(1.87±0.14)d,与对照组无统计学差异(P>0.05);切缘阳性率是3.33%(2/60),显著低于对照组(P<0.05);研究组患者的1月、3月、6月、9月、12月尿控率分别是25.00%(15/60)、53.33%(32/60)、71.67%(43/60)、91.67%(55/60)、98.33%(59/60),均显著高于对照组(P<0.05)。结论达芬奇机器人辅助腹腔镜前列腺癌根治术的手术操作时间短,术中出血少,术后恢复快,疗效好,值得临床推广应用。Objective To compare the therapeutic effects of ELRP and RALRP in patients under T2.Methods 128 patients with prostate cancer from January 2013 to December 2018 were randomly divided into a control group and a research group.The control group used ELRP and the study group used RALRP.The treatment effects of the two groups were compared.Results The bladder urethral anastomosis time in the study group was(18.75±1.42)min,indwelling drainage tube time was(5.14±0.47)d,the intraoperative blood loss was(94.27±9.05)mL,postoperative hospital stay was(5.27±0.48)d,positive rate of margin was 3.33%(2/60),the 1,3,6,9,12-month urinary control rates of the study group were 25.00%(15/60),53.33%(32/60),71.67%(43/60),91.67%(55/60),98.33%(59/60),which were significantly better than the control group(P<0.05).The operation time of the patients was(156.86±11.05)min,intestinal function recovery time was(1.87±0.14)d,no statistical difference from the control group.Conclusion The operation time of RALRP is shorter,the bleeding is less during the operation,and the recovery is faster after the operation.
