牙周牙髓联合治疗方案改善重度牙周炎疗效的作用

摘　　要：目的探讨重度牙周炎患者应用牙周牙髓联合治疗方案的治疗效果。方法研究选择2017年2月至2019年2月在大连市口腔医院口腔内科治疗的90例重度牙周炎患者为对象,随机分组分为对照组45例(牙周治疗);观察组45例(牙周牙髓联合治疗),对比两组治疗效果、临床相关指标。结果①观察组患者的治疗有效率(93.33%)高于对照组(73.33%)(P<0.05);②治疗前两组菌斑指数、牙周袋深度和龈沟出血指数对比无显著差异(P>0.05),治疗后观察组上述三项指标明显低于对照组(P<0.05);③治疗前,两组釉牙骨质界到骨缺陷的距离(CEJ-BD)与釉牙骨质界到牙槽嵴顶的距离(CEJ-AC)对比无差异(P>0.05),治疗后观察组CEJ-BD和CEJ-AC低于对照组(P<0.05)。结论采用牙周牙髓联合治疗重度牙周炎的效果显著,能够有效促进牙周组织的再生。Objective To explore the therapeutic effect of periodontal and endodontic therapy in patients with severe periodontitis.Methods The study selected 90 patients with severe periodontitis treated in the Department of Stomatology,Dalian Stomatological Hospital from February 2017 to February 2019,and randomly divided them into a control group of 45 patients(periodontal treatment);an observation group of 45 patients(dental peripheral endodontic treatment),compare the treatment effects and clinical related indicators of the two groups.Results(1)The effective rate of treatment in the observation group(93.33%)was higher than that of the control group(73.33%)(P<0.05).(2)There was no significant difference in the comparison of plaque index,periodontal pocket depth and gingival crevicular bleeding index between the two groups before treatment(P>0.05),the above three indicators in the observation group after treatment were significantly lower than those in the control group(P<0.05).(3)Before treatment,the distance between the enamel-cementum boundary and the bone defect(CEJ-BD)and the enamel-cementum boundary distance to the alveolar crest(CEJ-AC)had no difference(P>0.05).After treatment,CEJ-BD and CEJ-AC in the observation group were lower than those in the control group(P<0.05).Conclusion The combination of periodontal endodontic treatment is effective in treating severe periodontitis and can effectively promote the regeneration of periodontal tissue.
