血清AFP、CEA、CA125联合检测在原发性肝癌诊断中的应用价值

摘　　要：目的:研究血清甲胎蛋白(AFP)、癌胚抗原(CEA)、糖类抗原125(CA125)联合检测在原发性肝癌(PHC)诊断中的应用价值。方法:选择79例PHC患者设为观察组,70例良性肝病患者设为对照组。采集入选者空腹静脉血,采用化学发光法测定AFP、CEA、CA125水平。比较两组AFP、CEA、CA125水平,并统计AFP、CEA、CA125检测阳性率,计算AFP、CEA、CA125单独和联合检测诊断PHC的敏感度、特异度。结果:观察组AFP、CEA、CA125水平及AFP、CEA、CA125阳性检出率高于对照组,差异有统计学意义(P<0.05);AFP+CEA+CA125联合诊断敏感度(81.01%)、特异度(88.57%)高于各指标单项检测结果,差异有统计学意义(P<0.05)。结论:血清AFP、CEA、CA125为诊断PHC的有效指标,联合检测可提高诊断PHC的特异度及敏感度。Objective:To study the value of combined detection of serum alpha fetoprotein(AFP),carcinoembryonic antigen(CEA)and carbohydrate antigen 125(CA125)in the diagnosis of primary liver cancer(PHC).Methods:79 patients with PHC were selected as the observation group,and 70 patients with benign liver disease were selected as the control group.Fasting venous blood was collected and the levels of AFP,CEA and CA125 were determined by chemiluminescence method.The levels of AFP,CEA and CA125 were compared between the two groups,and the positive rates of AFP,CEA and CA125 were counted.The sensitivity and specificity of AFP,CEA and CA125 single detection and joint detection in the diagnosis of PHC were calculated.Results:The levels of AFP,CEA,CA125 and the positive detection rate of AFP,CEA and CA125 in the observation group were higher than those in the control group,the difference was statistically significant(P<0.05),the sensitivity(81.01%)and specificity(88.57%)of combined diagnosis of AFP+CEA+CA125 were higher than the single detection results of each index,and the difference was statistically significant(P<0.05).Conclusion:Serum AFP,CEA,CA125 are effective indicators for the diagnosis of PHC.Combined diagnosis can improve the specificity and sensitivity of PHC.
