坚固内固定术联合牵引复位固定术对口腔颌面部创伤性骨折患者并发症和预后的影响  

Effect of Rigid Internal Fixation Combined with Traction Reduction and Fixation on Complications and Prognosis of Patients with Oral and Maxillofacial Traumatic Fracture

摘　　要：目的:分析坚固内固定术联合牵引复位固定术治疗口腔颌面部创伤性骨折的临床价值。方法:将2015年1月-2020年4月笔者所在医院收治的80例口腔颌面部创伤性骨折患者作为研究对象,按照治疗方式的不同将其分为单一组(40例)和联合组(40例)。单一组行坚固内固定术治疗,联合组行坚固内固定术联合牵引复位固定术治疗。比较两组并发症发生情况、骨折愈合时间、术后住院时间及生活质量。结果:治疗后,联合组并发症发生率为2.50%,低于单一组的17.50%,差异有统计学意义(P<0.05)。联合组术后住院时间、骨折愈合时间分别为(9.04±2.04)d、(84.74±5.06)d,均优于单一组的(13.03±2.05)d、(122.43±6.03)d,差异有统计学意义(P<0.05)。联合组SF-36各评分高于单一组,差异有统计学意义(P<0.05)。结论:在口腔颌面部创伤性骨折的临床治疗中,采用坚固内固定术与牵引复位固定术的联合疗法能够显著降低并发症的发生风险,有利于提高预后,改善患者的生活质量。Objective:To analyze the clinical value of rigid internal fixation combined with traction reduction and fixation in the treatment of oral and maxillofacial traumatic fracture.Method:A total of 80 patients with oral and maxillofacial traumatic fracture admitted in our hospital from January 2015 to April 2020 were selected as the study subjects,and were divided into the single group (40 cases) and the combined group (40 cases) according to different treatment methods.The single group was treated with rigid internal fixation.The combined group was treated with rigid internal fixation combined with traction reduction and fixation.The incidence of complications,fracture healing time,postoperative hospital stay and quality of life were compared between the two groups.Result:After treatment,the incidence of complication in the combined group was 2.50%,which was lower than 17.50% in the single group,the difference was statistically significant (P<0.05).Postoperative hospital stay and fracture healing time in the combined group were (9.04±2.04) d and (84.74±5.06) d,respectively,which were better than (13.03±2.05) d and (122.43±6.03) d in the single group,the differences were statistically significant (P<0.05).The SF-36 score of the combined group was higher than that of the single group,and the difference was statistically significant (P<0.05).Conclusion:In the clinical treatment of oral and maxillofacial traumatic fracture,the combination of rigid internal fixation and traction reduction and fixation can significantly reduce the risk of complications,improve the prognosis and improve the quality of life of patients.
