摘　　要：非酒精性脂肪性肝炎(nonalcoholic steatohepatitis,NASH)是非酒精性脂肪性肝病(nonalcoholic fatty liver disease,NAFLD)疾病谱中更具危害性的阶段。脂毒性引发的肝细胞脂性凋亡不仅是NASH病程中肝细胞损伤的主要形式,而且还推动了NASH病程的进展。充分了解肝细胞脂性凋亡在NASH中的作用及机制,可为NASH的诊断与治疗提供新思路。本文将从直接效应和间接效应两方面阐述肝细胞脂性凋亡在NASH中的作用,并从脂性凋亡调控的效应分子、核心信号通路及其上游调控因子三个层面对肝细胞脂性凋亡分子机制做简要的综述。Nonalcoholic steatohepatitis(NASH)is a more aggressive stage in the spectrum of nonalcoholic fatty liver disease(NAFLD).Lipoapoptosis induced by lipotoxicity is the main form of hepatocyte injury during NASH procession.Moreover,it promotes the progress of NASH.Fully understanding the role and mechanism of lipoapoptosis in NASH may provide new ideas for the diagnosis and treatment of NASH.In this review,we will explain the direct and indirect effects of hepatocyte lipoapoptosis on NASH and summarize the molecular mechanism of hepatocyte lipoapoptosis from three aspects including effector molecules,main pathways and upstream regulators.
