内镜下两种“三明治”法治疗GOV2型EGV的临床应用研究

摘　　要：目的:探讨内镜下两种“三明治”法治疗GOV2型食管胃静脉曲张的临床疗效及安全性。方法:将医院消化科收住的89例患者采用随机分组的方式分为两组,实验组47例注射50%葡萄糖+组织胶+50%葡萄糖,对照组42例患者注射聚桂醇+组织胶+聚桂醇,分析两种方法的术中组织胶使用量、拔针出血率、术后曲张静脉改善/消失率、再出血率,以及排胶出血、狭窄等并发症发生的情况。结果:实验组患者术后曲张静脉改善率与对照组比较,差异无统计学意义(P>0.05)。而实验组患者各种并发症发生率明显低于对照组,差异具有统计学意义(P<0.05)。结论:内镜下注射“糖-胶-糖三明治”法较“聚-胶-聚三明治”治疗GOV2型食管胃静脉曲张更安全、经济,术后并发症更少,具有良好的临床应用价值,值得进一步研究。Objective:To investigate the clinical efficacy and safety of two"sandwich"under endoscope in the treatment of Type GOV2 esophageal and gastric varices.Methods:Eighty-nine patients with Type GOV2 esophageal and gastric varices admitted to the Hospital were randomly divided into two groups,experimental group of 47 cases of 50%glucose+tissue glue+50%glucose"sandwich",42 cases were gathered cinnamyl alcohol+tissue glue+cinnamyl alcohol"sandwich",To analyze the intraoperative use of tissue glue,bleeding rate of needle extraction,postoperative improvement,disappearance rate and rebleeding rate of varicose veins in GOV2 patients with esophageal and gastric varicose veins treated by injection of two"sandwich"methods,as well as the occurrence of complications such as bleeding from glue drainage and stenosis.Results:There was no significant difference in postoperative varicose vein improvement rate between the experimental group and the control group(P>0.05).The incidence of complications in the experimental group was significantly lower than that in the control group,with significant differences(P<0.05).Conclusion:Endoscopic injection of"sugar-glue-sugar sandwich"is safer and more economic than"poly-glue-poly-sandwich"in the treatment of GOV2 type esophageal and gastric varicose veins,with fewer postoperative complications.Therefore,it has good clinical application value and deserves further study.
