摘  要:目的检测肺癌患者血浆甲基化矮小同源盒基因2(SHOX2)及RAS相关区域家族1A(RASSF1A)基因,并探讨其意义。方法肺癌患者92例(观察组),肺部良性疾病患者68例(对照组),采用甲基化特异PCR法检测两组血浆甲基化SHOX2、RASSF1A基因,并计算甲基化SHOX2、RASSF1A基因阳性率;分析血浆SHOX2、RASSF1A基因甲基化阳性率与肺癌临床病理特征的关系;受试者工作特征(ROC)曲线分析血浆甲基化SHOX2、RASSF1A基因阳性对肺癌的诊断价值;另分析血浆与癌组织、支气管肺泡灌洗液(BALF)甲基化SHOX2、RASSF1A基因阳性率的一致性。结果与对照组比较,观察组血浆甲基化SHOX2、RASSF1A基因阳性率高(P均<0.05)。血浆甲基化SHOX2、RASSF1A基因阳性与肺癌肿瘤分期、淋巴结转移有关(P均<0.05),而与性别、年龄、病理类型、肿瘤分化程度无明显相关性(P均>0.05)。血浆甲基化SHOX2基因单独检测诊断肺癌的灵敏度为53.7%、特异度为86.07%,血浆甲基化RASSF1A基因单独检测诊断肺癌的灵敏度为64.1%、特异度为92.1%,两者联合检测诊断肺癌的灵敏度为81.6%、特异度为94.2%。血浆与癌组织、BALF中甲基化SHOX2基因阳性率具有良好的一致性,一致率分别为87.3%(48/55)、97.9%(47/48),血浆与癌组织、BALF中甲基化RASSF1A基因阳性率具有良好的一致性,一致率分别为86.4%(51/59)、96.1%(49/51)。结论肺癌血浆甲基化SHOX2、RASSF1A基因阳性率增加,两指标与肺癌肿瘤分期、淋巴结转移有关,联合检测两者有助于提高肺癌的诊断,血浆与癌组织、BALF中甲基化SHOX2、RASSF1A基因阳性率具有良好的一致性。Objective To observe the methylated short homeobox gene 2(SHOX2)and RAS-related region family 1 A(RASSF1 A)genes in plasma of lung cancer patients and to explore their significance.Methods There were 92 cases of patients with lung cancer(observation group)and 68 cases of patients with benign lung disease(control group).The methylated SHOX2 and RASSF1 A genes were detected by methylation-specific PCR,and the positive rates of methylated SHOX2 and RASSF1 A genes were calculated.The consistency of methylated SHOX2 and RASSF1 A gene positive rate in the plasma,cancer tissues,bronchoalveolar lavage fluid(BALF)and the relationship between SHOX2,RASSF1 A gene methylation positive rate and clinical pathological characteristics of lung cancer were analyzed statistically.The receiver operating characteristic(ROC)curve was used to analyze the value of positive rates of methylated SHOX2 and RASSF1 A genes in diagnosis of lung cancer.Results Compared with the control group,the positive rates of methylated SHOX2 and RASSF1 A genes in the observation group were higher(both P<0.05).The positive rates of plasma methylated SHOX2 and RASSF1 A genes were related to tumor stage and lymph node metastasis(both P<0.05),but not related to gender,age,pathological type,or tumor differentiation(all P>0.05).The sensitivity and specificity of methylated SHOX2 gene in the diagnosis of lung cancer were 53.7%and 86.07%,respectively;the sensitivity and specificity of methylated RASSF1 A in the diagnosis of lung cancer were 64.1%and 92.1%,respectively;the sensitivity and specificity of the combined detection in the diagnosis of lung cancer were 81.6%and 94.2%,respectively.The positive rates of methylated SHOX2 gene in plasma and cancer tissues and BALF were 87.3%(48/55)and 97.9%(47/48),respectively,and the positive rates of methylated RASSF1 A gene in the plasma and cancer tissues and BALF were 86.4%(51/59)and 96.1%(49/51),respectively.Conclusions The positive rates of methylated SHOX2 and RASSF1 A genes of lung cancer increase,and the two indexes
