摘　　要：目的:探讨问题解决疗法对糖尿病视网膜病变合并心理痛苦患者的影响,为该类患者的护理干预提供初步证据。方法:选择某三级甲等医院糖尿病视网膜病变合并心理痛苦患者50例,使用信封法分为实验组和对照组,对照组给予常规健康指导,实验组在对照组基础上,实施问题解决疗法为基础的护理干预。分别在干预前、干预后3个月、干预后6个月评估患者的心理痛苦得分、抑郁症状得分、自我管理得分和糖化血红蛋白(HbA1c)的变化情况。结果:干预后3个月、6个月,实验组抑郁症状得分低于对照组(t=-2.056,-3.023;P<0.05);实验组自我管理得分高于对照组(t=2.089,2.029;P<0.05);干预后6个月,实验组糖尿病心理痛苦得分和糖化血红蛋白(HbA1c)低于对照组(t=-2.020,-2.267;P<0.05)。结论:问题解决疗法能够改善患者的心理痛苦和血糖状况。Objective:To investigate the influence of problem-solving therapy on patients with diabetic retinopathy(DR)and psychological distress.Methods:A total of 50 participants with DR and psychological distress were allocated in a third-grade class-A hospital,and were randomly assigned into experimental group and control group,25 couples each.The control group was applied with only routine knowledge guidance.The experimental group was applied with nursing intervention based on problem-solving therapy.Respectively before the intervention,at 3 months and 6 months after intervention,evaluation in patients’psychological distress score,depression score,self-management scores and the change of glycated hemoglobin(HbA1 c)were evaluated.Results:At 3 months and 6 months after intervention,the experimental group were lower than the control group in depression score(t=-2.056,-3.023;P<0.05),and higher in self-management score(t=2.089,2.029,P<0.05);At 6 months after intervention,the psychological distress scores and glycated hemoglobin(HbA1 c)in the experimental diabetes were lower than those in the control group(t=-2.020,-2.267;P<0.05).Conclusion:Problem-solving therapy is a promising intervention for improving psychological distress and blood glucose.
