枣仁安神胶囊联合碳酸锂对双相情感障碍维持期失眠患者睡眠质量及生活质量的影响

摘　　要：目的:观察枣仁安神胶囊联合碳酸锂对双相情感障碍维持期失眠患者睡眠质量及生活质量的影响。方法:将138例心胆气虚型双相情感障碍维持失眠患者随机分为观察组与对照组各69例,对照组给予碳酸锂缓释片治疗,观察组给予枣仁安神胶囊联合碳酸锂缓释片治疗。2组均连续治疗2个疗程,1个疗程为4周。治疗前后应用匹茨堡睡眠质量指数(PSQI)评定患者的睡眠质量,应用躁狂量表(BRMS)评价躁狂状态,应用抑郁自评量表(SDS)与焦虑自评量表(SAS)评价焦虑、抑郁情况,应用生活质量评价量表(SF-36)评价生活质量,并对比治疗后2组的临床疗效及不良反应情况。结果:治疗后,观察组总有效率92.75%,对照组总有效率73.91%,2组比较,差异有统计学意义(P<0.05)。2组BRMS、SAS、SDS评分及PSQI均较治疗前下降,观察组各指标值均低于对照组,差异均有统计学意义(P<0.05)。2组SF-36各项生活质量评分均较治疗前升高,观察组各项生活质量评分均高于对照组,差异均有统计学意义(P<0.05)。结论:采用枣仁安神胶囊联合碳酸锂治疗双相情感障碍维持期失眠患者疗效确切,可有效改善失眠,同时改善焦虑、抑郁等负向心理,从而提高患者的生活质量,且不良反应少。Objective:To observe the effect of Zaoren Anshen capsules combined with lithium carbonate on sleep quality and quality of life in patients with insomnia in maintenance period of bipolar affective disorder.Methods:A total of 138 cases of patients with insomnia of heart-gallbladder qi deficiency type in maintenance period of bipolar affective disorder were randomly divided into the observation group and the control group,69 cases in each group.The control group was treated with lithium carbonate sustained-release tablets,and the observation group was additionally treated with Zaoren Anshen capsules based on the treatment of the control group.Both groups were treated for two courses,four weeks being a course.Before and after treatment,sleep quality of patient was evaluated by Pittsburgh Sleep Quality Index(PSQI),the state of mania by Bech-Rafaelsen Mania Scale(BRMS),anxiety and depression by Self-rating Depression Scale(SDS)and Self-rating Anxiety Scale(SAS),and quality of life by 36-Item Short Form Health Survey(SF-36).After treatment,the clinical effect and incidences of adverse reactions in the two groups were compared.Results:After treatment,the total effective rate was92.75%in the observation group and 73.91%in the control group,the difference being significant(P<0.05).The scores of BRMS,SAS,SDS and PSQI in the two groups were decreased when compared with those before treatment,and the indexes in the observation group were lower than those in the control group,differences being significant(P<0.05).The scores of quality of life in SF-36 in the two groups were higher than those before treatment,and the eight scores of quality of life in the observation group were higher than those in the control group,differences being significant(P<0.05).Conclusion:The application of Zaoren Anshen capsules combined with lithium carbonate has definite curative effect in treating patients with insomnia in maintenance period of bipolar affective disorder,and can effectively improve insomnia,anxiety,depression and other negative emo
