新型血清标志物应用于细菌性肝脓肿诊断中的前瞻性研究

摘　　要：目的对血清标志物铁调素、可溶性CD14亚型的应用进行前瞻性研究,评价其单独或联合应用于细菌性肝脓肿诊断的价值。方法选取哈尔滨医科大学附属第一-医院2019年6月至12月期间住院治疗的细菌性肝脓肿患者30名作为研究组,同期招募健康志愿者共40人作为对照组,分别采集血清样本,检测其中铁调素、可溶性CD14亚型及其他相关指标,.使用统计软件SPSS25进行分析。结果铁调素、可溶性CD14亚型在患者组样本中的浓度显著高于对照组,以受试者工作特征(ROC)曲线分析,分别取截断值为52.56ng/mL.5.50ng/mL,此时诊断敏感度为92%.73%,特异度为91%.97%,以二元Logistic回归分析二者联合因子,可达敏感度95%、特异度97%。以序贯性器官功能衰竭评分作为严重程度指标,各标志物均无显著相关性;针对病程中的不良事件进行分组,组间无显著差异性。结论单独或联合应用铁调素索与可溶性CD14亚型对于细菌性肝脓肿的临床诊疗有辅助诊断价值。Objective This research aims to evaluate the value of serum biomarkers called hepeidin and presepsin(solubl-eCD14 subtype,sCD14-ST,presepsin)in clinical diagnosis of pyogenic liver abscess.Methods This research has 30 inpatients of py-ogenic liver abscess in the First Affiliated Hospital of Harbin Medical University from June 2019 to December 2019 as the research group and 40 healthy volunteers as the control group.Serum levels of hepcidin and presepsin were tested and analyzed with other infor-mation.Results Levels of biomarkers were higher in patients’serum sigmificantly.Analyzed by receiver operating characteristic(ROC)curve,the cut-off value of hepcidin was 52.56 ng/ml and presepsin was 5.50 ng/ml.The sensitivity of Hepcidin was 92%and presepsin was 73%.The specifieity of Hepeidin was 91%and 97%.The sensitivity and specificity of hepcidin and presepsin com-bined diagnosis were 95%and 97%.The areas under the curve(AUC)of them were 0.945,0.909,0.990.To measure the SOFA score and the biomarkers,there was no significant correlation.There was no signifcant difference of serum biomarkers between the groups of different prognosis.Conclusion The single and combined detection of hepeidin and presepsin has a significant value in the clinical diagnosis of pyogenic liver abscess.
