经半-半椎板显微手术切除椎管内髓外膜内肿瘤临床疗效

摘　　要：目的探讨经一侧半-半椎板入路显微手术在切除椎管内髓外膜内肿瘤的临床疗效。方法回顾性分析2016年7月至2019年6月收治的18例经一侧半-半椎板显微手术切除椎管内髓外膜内肿瘤的病例资料。其中神经鞘瘤12例,脊膜瘤4例,神经纤维瘤1例,室管膜瘤1例。结果18例患者肿瘤均完全切除,术后5d后即可下床活动。随访6~24个月,患者症状和体征均明显改善,无明显手术并发症和脊柱畸形,所有病例肿瘤未见复发。结论经一侧半-半椎板显微入路,能够最大程度保留脊柱后部的骨结构,有利于保持脊柱稳定性,具有创伤小、出血少、安全、卧床时间短等优点,值得临床推广应用。Objective To investigate the clinical efficacy of treating spinal intradural extramedullary tumors through hemi-semi-laminectomy approach.Methods The clinical data of 18 patients with spinal intradural extramedullary tumors treated through hemi-semi-laminectomy approach were retrospectively analyzed in our department from July 2016 to June 2019,including 12 schwannomas,4 meningiomas,1 neurofibroma,1 astrocytomas.Results All the tumors in 18 patients were totally removed.The patients who were opratated can move off bed after 5 days.All the patients were followed up for 6 to 24 months,the symptoms and signs were significantly improved,and there was no obvious surgical complications and spinal deformity.There was no tumor relapse.Conclusion Hemi-semi-laminectomy micro surgery for the removal of the tumors in the spinal,which can hold the bone structrue of post spine at most and proft the stability of spinal,has such advantages as minimal invasion,less bleeding,safety and short time of bed rest.So this method is worth promoting and applying.
