颅咽管瘤手术患者预后影响因素分析

摘　　要：目的探讨影响颅咽管瘤(CP)手术患者预后的相关因素,为临床个性化手术治疗及预后评估提供帮助。方法收集青岛大学附属医院神经外科2014年6月—2019年5月CP手术患者的一般资料,采用SPSS软件分析CP手术患者预后的影响因素。结果单因素分析显示,肿瘤直径、质地、QST分型、黏附强度分型、切除程度以及病理分型与患者预后明显相关(H=6.542~36.502,Z=-2.914,P<0.05);多因素Logistic回归分析显示,黏附强度分型(OR=0.001~0.059,95%CI=0.001~0.464,χ^2=7.255~16.895,P<0.01)、QST分型(OR=0.016~0.167,95%CI=0.002~0.821,χ^2=4.850~12.968,P<0.05)是影响CP手术患者预后的独立危险因素。结论黏附强度及QST分型是CP手术患者预后的重要影响因素,术前充分了解CP的黏附强度及QST分型对于患者手术策略、治疗方案的制定以及预后评估具有重要的参考价值及指导意义。Objective\To investigate the influencing factors for the prognosis of patients undergoing surgery for craniopharyngioma(CP),and to provide help for individualized surgical treatment and prognostic evaluation.\Methods\General data were collected from the patients with CP who underwent surgery in Department of Neurosurgery in The Affiliated Hospital of Qingdao University from June 2014 to may 2019,and SPSS software was used to analyze the influencing factors for the prognosis of patients with CP after surgery.\Results\The univariate analysis showed that tumor diameter,tumor texture,QST classification,adhesion intensity classification,extent of resection,and pathological classification were associated with prognosis(H=6.542-36.502,Z=-2.914,P<0.05),and the multivariate logistic regression analysis showed that adhesion intensity classification(OR=0.001-0.059,95%CI=0.001-0.464,χ^2=7.255-16.895,P<0.01)and QST classification(OR=0.016-0.167,95%CI=0.002-0.821,χ^2=4.850-12.968,P<0.05)were independent risk factors for the prognosis of patients after surgery for CP.\Conclusion\Adhesion intensity classification and QST classification are important influencing factors for the prognosis of patients undergoing surgery for CP,and a full understanding of the adhesion intensity classification and QST classification of CP before surgery has important reference value and guiding significance in the formulation of surgical strategy and treatment plan and the evaluation of prognosis.
