独活寄生汤联合唑来磷酸治疗恶性肿瘤骨转移的临床疗效

摘　　要：目的:观察独活寄生汤联合唑来磷酸治疗肝肾亏虚型恶性肿瘤骨转移患者的临床疗效。方法:选择恶性肿瘤骨转移肝肾亏虚型患者110例,以随机抽签法将其分为对照组(53例)与观察组(57例)。对照组与观察组患者在年龄、性别、原发疾病、分期等方面差异无统计学意义(P>0.05)。对照组在常规营养支持及止痛治疗的基础上给予唑来磷酸治疗,观察组在对照组基础上加用独活寄生汤口服。观察两组治疗前后的生活质量评分、疼痛数字评价量表(Numerical Rating Scale,NRS)、生化指标、不良反应的差异。结果:治疗后,观察组在总体生活质量、症状领域、功能领域各维度评分明显高于对照组(P<0.05),疼痛评分、镇痛起效时间、疼痛持续时间的改善优于对照组(P<0.05);血钙、血清癌胚抗原(Carcinoembryonic Antigen,CEA)、血红蛋白(Hemoglobin,Hb)、血小板(Platelet,PLT)及白细胞(White Blood Cell,WBC)水平改善效果优于对照组(P<0.05);观察组所致的不良反应总发生例数低于对照组。结论:独活寄生汤联合唑来磷酸治疗肝肾亏虚型恶性肿瘤骨转移患者可明显改善其生活质量与生化指标,且缓解疼痛效果更显著,优于单纯应用唑来磷酸患者。Objective:To observe the clinical efficacy of Duhuo Jisheng decoction(独活寄生汤)combined with zoledronic acid in the treatment of bone metastasis of malignant tumor.Methods:110 cases of malignant tumor bone metastases were selected as the research subjects and randomly divided into the experimental group(53 cases)and the control group(53 cases).There were no significant differences in age,sex,primary disease and stage between the control group and the observation group.The control group was given zoledronic acid on the basis of routine nutritional support and analgesic treatment,and the observation group was given Duhuo Jisheng decoction orally on the basis of treatment of the control group.After one course of treatment,the quality of life,NRS score,biochemical indicators and untoward reactions were compared between the 2 groups.Results:After one course of treatment,the scores of overall quality of life,symptom and function in the observation group were significantly higher than those in the control group(P<0.05),and the improvement of pain score,Analgesia onset time and pain duration were better than those in the control group(P<0.05).The biochemical indicators in the experimental group were significantly greater than those of the control group and the difference was statistically significant(P<0.05).During the treatment,the total number of adverse reactions in the observation group was lower than that in the control group.Conclusion:The combination of Duhuo Jisheng decoction and zoledronic acid can obviously improve the quality of life and biochemical indexes of patients with bone metastasis of deficiency of liver and kidney type malignant tumor,and the effect of relieving pain is more remarkable than zoledronic acid alone.
