摘 要:目的观察含ω-3多不饱和脂肪酸(polyunsaturated fatty acids,PUFAs)的肠内营养对胃癌患者术后营养状况及生活质量的影响。方法选择门诊确诊为单纯性局部进展期胃癌并拟行远端胃癌D2根治术和常规化疗的患者60例,随机分为ω-3 PUFAs肠内营养干预组(ω-3组)和常规肠内营养干预组(对照组)。两组均在术后启动肠内营养时开始干预,持续3个月,比较两组患者干预前和干预结束时的营养状况、炎症指标、生活质量评分等。结果共有52例患者完成研究,ω-3组25例,对照组27例,干预后ω-3组患者血液中ω-3 PUFAs、ω-6 PUFAs浓度较对照组和干预前均显著升高(P<0.01),C反应蛋白显著降低(P<0.05),维生素B12浓度较对照组显著升高(P<0.05),体重、体重指数显著高于对照组(P<0.05),能量和蛋白质摄入、生活质量评分均优于对照组(P<0.01)。结论胃癌患者术后恢复期进行富含ω-3 PUFAs的肠内营养治疗,有助于改善其营养状况,降低炎症反应,提高生活质量。Objective To observe the effect of enteral nutrition containingω-3 polyunsaturated fatty acids(PUFAs)on the nutritional status and quality of life of patients after radical surgery.Methods A total of 60 locally advanced gastric cancer patients undergoing D2 radical surgery and standard chemotherapy were selected and randomly divided intoω-3 group(receiving enteral nutrition containingω-3 PUFAs)and control group(receiving routine enteral nutrition intervention).The intervention was initiated at the time of postoperative enteral nutrition initiation and lasted for 3 months.Then we compared the nutritional status,inflammation index,quality of life scores before and after the intervention between the two groups.Results 52 patients completed the trial,including 25 in theω-3 group and 27 in the control group.After intervention,the bloodω-3 andω-6 PUFAs levels of theω-3 group were significantly higher and the C-reactive protein were reduced than those of the control group and before intervention(P<0.01),and the concentration of vitamin B12 of theω-3 group was significantly higher than that of the control group(P<0.05).Body weight and body mass index of theω-3 group were significantly higher than those of the control group(P<0.05),and the energy and protein intake,the quality of life scores of theω-3 group were better than those of the control group(P<0.01).Conclusion Enteral nutrition support containingω-3 PUFAs is helpful to improve the nutritional status,reduce the inflammatory response and improve the quality of life of patients with gastric cancer in the recovery period.
