EGFR、VEGF和Ki67在食管癌中的表达及其临床意义

摘　　要：目的通过检测EGFR、VEGF、Ki67蛋白在食管癌组织中的表达,分析其与食管癌临床病理特征的关系,为食管癌的诊治和预后评估提供理论依据。方法收集经病理确诊的104例食管鳞癌患者的临床资料,采用免疫组织化学染色法检测其癌组织标本、癌旁组织标本中表皮生长因子受体(EGFR)、血管内皮生长因子(VEGF)、细胞核相关抗原(Ki67)的蛋白表达水平,分析癌组织中EGFR、VEGF、Ki67表达水平与患者临床病理特征的关系及三种蛋白之间表达的相关性。结果食管癌组织标本中EGFR、VEGF、Ki67表达阳性率均高于癌旁组织标本,差异均有统计学意义(P<0.05);Spearman相关分析显示,食管癌患者癌组织EGFR表达阳性率与VEGF表达阳性率呈正相关(P<0.05),EGFR表达阳性率与Ki67表达阳性率呈正相关(P<0.05),VEGF表达阳性率与Ki67表达阳性率无相关性(P>0.05);食管癌患者EGFR表达水平与肿瘤分化程度、浸润深度有关(P<0.05);VEGF表达水平与肿瘤分化程度、浸润深度、淋巴结转移有关(P<0.05);Ki67表达水平与性别、年龄、肿瘤大小、远处转移、分化程度、浸润深度、淋巴结转移均无关(P>0.05)。结论食管癌患者癌组织中EGFR、VEGF、Ki67蛋白呈高表达,其中EGFR、VEGF的表达与肿瘤的分化程度、浸润深度呈正相关,可能参与食管癌发生、发展过程,对于食管癌的诊治及预后评估具有临床参考意义。Objective To explore the relationship between expression of EGFR,VEGF and ki67 and clinicopathological characteristics of esophageal cancer,and to provides theoretical basis for the diagnosis,treatment and prognosis of esophageal cancer.Methods 104 patients with esophageal squamous cell carcinoma diagnosed by pathology were collected.The protein expression levels of EGFR,VEGF and Ki67 in tissue samples were analyzed.The relationship between the expression levels of EGFR,VEGF and Ki67 in cancer tissues and the clinical pathological characteristics of patients and the correlation among the expressions of the three proteins were analyzed.Results The positive expression rates of EGFR,VEGF and Ki67 in esophageal cancer tissue samples were higher than those in normal esophageal mucosa tissue near the cancer(P<0.05);Spearman correlation analysis showed that the positive rate of EGFR expression in esophageal cancer patients was positively correlated with the positive rate of VEGF expression(P<0.05);The positive rate of EGFR expression was positively correlated with the positive rate of Ki67 expression(P<0.05);The positive rate of VEGF expression was not correlated with the positive rate of Ki67 expression(P>0.05);The EGFR expression level of patients with esophageal cancer was related to the degree of differentiation and depth of invasion(P<0.05);VEGF expression level was related to the degree of differentiation,depth of invasion and lymph node metastasis(P<0.05);Ki67 expression level was related to distant metastasis,degree of differentiation,depth of infiltration and lymph node metastasis were not related(P>0.05).Conclusion The EGFR,VEGF and Ki67 proteins are highly expressed in cancer tissues of patients with esophageal cancer.The expression of EGFR and VEGF are positively correlated with tumor differentiation and depth of invasion.They may be involved in the development and progression of esophageal cancer,and there are reference significances in diagnosis,treatment and prognosis of patients with esophageal cancer.
