中医护理干预脑卒中后吞咽障碍疗效的Meta分析

摘　　要：目的通过Meta分析系统评价中医护理对脑卒中后吞咽障碍的治疗效果。方法计算机检索知网、中国生物医学文献数据库、万方、维普、PubMed等多个中、英文数据库,收集有关中医护理干预脑卒中吞咽障碍的随机对照试验,检索时限从建库至2020年7月1日。对符合纳排标准的文献,采用Rev Man 5.3软件进行Meta分析。结果总共纳入10篇文献,包括921例患者。Meta分析结果显示:中医护理较常规护理对吞咽功能的有效率有提高作用,合并效应具有统计学意义[OR=6.34,95%CI(3.94,10.20),P<0.00001];中医护理能有效降低洼田饮水试验评分[SMD=-2.82,95%CI(-3.13,-2.52),P<0.00001]及相关并发症的发生率[OR=0.27,95%CI(0.12,0.58),P=0.001];中医护理能增加患者的反复唾液吞咽次数[MD=0.60,95%CI(0.39,0.81),P<0.00001]。结论中医护理有助于改善脑卒中吞咽障碍患者的吞咽功能,可促进患者康复,利于患者预后。Objective To systematically evaluate the effect of traditional Chinese medicine(TCM)nursing on the treatment of dysphagia after stroke by Meta-analysis.Methods Computer search was conducted on CNKI,Chinese biomedical literature database,Wanfang,Weipu,PubMed and other databases to collect randomized controlled trials on TCM nursing intervention for stroke dysphagia,and the retrieval period was from Establish a database to July 1,2020.Rev Man 5.3 was used for data analysis of literature to meet the inclusion standards.Results A total of 10 articles were included,including 921 patients.Meta-analysis results showed that TCM nursing improved the effective rate of swallowing function compared with conventional nursing,and the combined effect was statistically significant[OR=6.34,95%CI(3.94,10.20),P<0.00001].TCM nursing could effectively reduce the score of drinking water test in low-lying fields[SMD=-2.82,95%CI(-3.13,-2.52),P<0.00001].TCM nursing could reduce the incidence of related complications[OR=0.27,95%CI(0.12,0.58),P=0.001].TCM nursing can increase the number of repeated salivary swallowing of patients[MD=0.60,95%CI(0.39,0.81),P<0.00001].Conclusion TCM nursing can improve the swallowing function of stroke patients with dysphagia and promote the rehabilitation of patients.
