摘　　要：目的探讨卵巢甲状腺肿的临床特点、诊断和鉴别诊断。方法汇报1例青岛大学附属医院妇科诊治的卵巢甲状腺肿合并大量腹水和CA125升高的病例,并进行文献复习及总结。结果卵巢甲状腺肿发病率低,临床工作中对该疾病的认识不足,尤其是伴发腹水及肿瘤标志物如CA125升高时,容易误诊为卵巢恶性肿瘤。CT影像学检查具有一定的特点,可为临床医生提供初步的诊断依据;诊断困难时可行腹腔镜探查,并根据术中冰冻病理结果,调整相应的治疗方案,最终诊断依据为石蜡病理结果。结论卵巢甲状腺肿发病率低,易发生误诊,临床医生应重视该疾病的特点,加强认识,提高术前诊断率。Objective to investigate the clinical characteristics,diagnosis and differential diagnosis of ovarian goiter.Methods A case of ovarian goiter with massive ascites and elevated CA125 was reported in the Affiliated Hospital of Qingdao University.Results The incidence rate of ovarian goiter is low,and the knowledge of this disease is insufficient in clinical work.Especially when accompanied with ascites and tumor markers such as CA125,it is easy misdiagnosed as ovarian malignant tumor.CT imaging examination of ovarian goiter has certain characteristics,which can provide a preliminary diagnostic basis for clinicians,Laparoscopic exploration is feasible when the diagnosis is difficult,and adjust the corresponding treatment plan according to intraoperative frozen pathological results,and the final diagnosis is based on paraffin pathological results.Conclusions The incidence rate of goiter is low,and it is easy to be misdiagnosed.Clinicians should pay attention to the characteristics of this disease,enhance their understanding and improve the preoperative diagnostic rate.
