上睑提肌缩短联合自体脂肪填充治疗外伤性上睑下垂伴凹陷临床效果分析

摘　　要：目的探讨上睑提肌缩短联合自体脂肪填充治疗外伤性上睑下垂伴凹陷的临床效果。方法收集我院2018年1月-2019年6月收治外伤性轻-重度上睑下垂伴上睑凹陷52例(52只眼),予行上睑提肌缩短联合自体脂肪填充,随访1年,并观察其上睑下垂、上睑闭眼、局部凹陷改善情况并计算其有效率。结果本组共52例52只眼均于术后7天拆线,切口均一期愈合,术前、术后6月及术后1年上睑缘遮盖角膜距离分别为(4.6±1.3mm)、(1.5±0.5mm)、(1.8±0.6mm),上睑局部无凹陷,随访至12个月,完全矫正46只,矫正良好6只,所有患者对其纠正效果满意,所有病例未见感染、暴露性角膜炎、睑内翻、结膜脱垂等并发症。结论上睑提肌缩短联合自体脂肪填充治疗外伤性上睑下垂伴凹陷其临床疗效佳,提肌缩短能明显改善其上睑下垂畸形,结合自体脂肪填充,也改善了因外伤导致的上睑凹陷。Objective To investigate the effect of levator palpebrae superioris shortening combined with autogenous fat transplantation in treatment of traumatic blepharoptosis with sagging upper eyelid.Methods From January 2018 to June 2019,52cases(52eyes)of traumatic blepharoptosis with sagging upper eyelid were treated with levator palpebrae superioris shortening combined with autogenous fat transplantation,all patients were followed up 1 year and observed the improvement of ptosis,eyelid closure and sagging upper eyelid,and calculated the effective rate.Results The sutures were removed 7 days postoperative,and the incisions healed at the first stage.The distance of cornea covered by upper eyelid margin was(4.6±1.3mm),(1.5±0.5mm)and(1.8±0.6mm)respectively preoperative,6 months and 1 year after operation,46 cases were cured and 6 cases were improved,there were no local depression in the upper eyelid and no infection,exposure keratitis,entropion,conjunctival prolapse and other complications were found in all cases.Conclusion Levator palpebrae superioris shortening combined with autogenous fat transplantation in treatment of traumatic blepharoptosis with sagging upper eyelid has a good effect.Levator palpebrae superioris shortening can significantly improve the ptosis deformity,combined with autogenous fat transplantation can also improve the sagging upper eyelid caused by trauma.
