双链法正畸矫治治疗不同上下颌牙列间隙患者的短期随访研究

摘　　要：目的探究双链法正畸矫治治疗不同上下颌牙列间隙患者的短期随访效果。方法选取2018年5月~2019年10月我院98例不同上下颌牙列间隙患者,根据治疗方案不同分组,各49例。对照组给予微种植体支抗配合直丝弓矫治,观察组给予双链法正畸矫治治疗。对比两组尖牙牙冠发生远中舌向扭转情况、牙间隙关闭时间、并发症、矫治前后上下颌槽间距之和及矫治前、矫治后3个月、矫治后6个月牙周情况[牙龈指数(GI)、龈沟出血指数(SBI)、龈沟探诊深度(SPD)、菌斑指数(PLI)]、龈沟液肿瘤坏死因子α(TNF-α)、白介素-1β(IL-1β)表达。结果观察组尖牙牙冠发生远中舌向扭转率低于对照组,牙间隙关闭时间短于对照组(P<0.05);矫治后两组上、下颌槽间距之和均低于矫治前(P<0.05),但组间对比差异无统计学意义(P>0.05);矫治后3个月、矫治后6个月两组GI、SBI、SPD、PLI高于矫治前(P<0.05);矫治后3个月、矫治后6个月观察组GI、PLI低于对照组(P<0.05);矫治后3个月、矫治后6个月两组龈沟液TNF-α、IL-1β表达高于矫治前,但观察组低于对照组(P<0.05);观察组并发症发生率2.04%低于对照组16.33%(P<0.05)。结论双链法正畸矫治治疗不同上下颌牙列间隙患者,可缩短牙间隙关闭时间,促进牙周组织健康,减少并发症。Objective To explore the short-term follow-up effect of double chain orthodontic treatment in patients with different maxillary and mandibular dentition spaces.Methods A total of 98 patients with different maxillary and mandibular dentition spaces in our hospital from May 2018 to October 2019 were selected and group them according to different treatment plans,with 49 cases in each group.The control group was treated with micro-implant anchorage combined with straight wire arch treatment,and the observation group was treated with double-strand orthodontic treatment.The distal lingual torsion of canine crown,closing time of tooth space and complication,the sum of the distance between the upper and lower jaw grooves before and after treatment,periodontal status[gingival index(GI),sulcus bleeding index(SBI),sulcus probing depth(SPD),plaque index(PLI)],gingival crevicular fluid tumor necrosis factorα(TNF-α)and interleukin-1β(IL-1β)expression before treatment,3 months after treatment,and 6 months after treatment were compared between the two groups.Results The rate of distal tongue torsion in the canine crown of the observation group was lower than that of the control group,and the closing time of the interdental space was shorter than that of the control group(P<0.05);after correction,the sum of the upper and mandibular groove distances between the two groups was lower than before correction(P<0.05),but there was no statistically significant difference between the groups(P>0.05);the GI,SBI,SPD and PLI of the two groups were higher than before the treatment at 3 months and 6 months after the treatment(P<0.05);the GI and PLI of the observation group were lower than those of the control group 3 months after treatment and 6 months after treatment(P<0.05);the expressions of TNF-αand IL-1βin the gingival crevicular fluid of the two groups were higher than before the treatment at 3 months and 6 months after the treatment,but the observation group was lower than the control group(P<0.05);the complication rate of the obs
