乳晕切口腺体免缝合式乳房纤维腺瘤切除术临床效果分析

摘　　要：目的探讨乳腺纤维腺瘤切除术后不予缝合腺体、留置创腔、放置皮片引流方法的临床效果。方法回顾性分析2017年1月至2019年12月收治的130例行乳腺纤维腺瘤切除术患者的临床资料。根据肿块切除后腺体处理方式分为两组:观察组77例,乳腺纤维腺瘤切除术后采用不予缝合腺体、留置创腔、放置皮片引流的方法处理;对照组53例,采用传统缝合腺体方式处理。比较两组手术情况,切口拆线时间,术后并发症,术后3个月乳房外观美容效果及主观评分、患者满意度及切口瘢痕情况。结果两组患者均无切口感染。与对照组比较,观察组手术时间短,切口拆线早,术后疼痛轻,术后并发症(局部血肿、皮下瘀斑、疼痛情况)少,术后乳房外观形态美容效果、主观评分、术后患者满意度及切口瘢痕情况均优于对照组(P<0.05)。结论乳腺纤维腺瘤切除术后不缝合腺体、留置创腔并放置皮片引流的处理并不会增加血肿、皮下瘀斑、切口延期愈合的风险,术后疼痛轻微,切口愈合更快,乳房外形美容效果更好,患者满意度较高,适用于直径小于4 cm的各种良性病损的切除,值得临床推广。Objective To investigate the clinical effect of gland suture-free,wound cavity indwelling and skin graft drainage during the resection of breast fibroadenoma.Methods The clinical data of 130 women with breast fibroadenoma performed mess resection from January 2017 to December 2019 were analyzed retrospectively.According to the management of gland after fibroadenoma resection,all cases were divided into two groups:77 cases were treated by suture-free,wound cavity indwelling and skin graft drainage as the observation group,and 53 cases were treated with traditional gland suturing as the control group.The operative outcomes,removal time of stitches,postoperative complications,breast cosmetic appearance effect and subjective score,postoperative patient satisfaction and incision scar condition 3 months after operation were compared between the two groups.ResultsCompare to the control group,the operation time was shorter,the stitch removal time was earlier,the VAS score was lower,the incidence of complication was less,and the postoperative breast cosmetic effect,subjective score of breast aesthetics,scar healing 3 months after the surgery and the satisfaction rate were higher in the observation group,and all the differences were statistically(P<0.05).Conclusions The management of breast gland after fibroadenoma resection with gland suturing-free,wound cavity indwelling and skin graft drainage placing will not increase the risk of hematoma,subcutaneous ecchymosis and wound delayed healing,but the patients can achieve benefits from it with milder pain,faster healing of the incision,ideal cosmetic effect and higher satisfaction.It is suitable for benign lesion resection,and which is worthy of clinical promotion.
