SMILE与FS-LASIK矫正近视术后像差分析

摘　　要：目的分析SMILE和飞秒激光制瓣LASIK矫正近视对角膜像差的影响。方法选取2018年10月-2019年8月在笔者医院屈光门诊分别实施Smile和飞秒激光制瓣LASIK的患者各200例;接受SMILE手术者为观察组(A组);接受飞秒激光制瓣LASIK为对照组(B组);分别测量术前、术后1个月、3个月、6个月时的高阶像差、球差、彗差、三叶草差。结果 (1)术后2组各像差都有所增加,与术前比较差异有统计学意义;(2)术后像差增幅:SMILE组各像差增幅均低于FS-LASIK组;①术后1M高阶像差、彗差、球差增幅差异有统计学意义(P<0.01);②三叶草差增幅在术后差异无统计学意义(P>0.05);③各像差及增幅随时间逐渐减少。结论 SMILE和FS-LASIK术后均引入新的像差,SMILE术后像差增加较少。Objective To analyse the effects of Smile and femtosecond laser flap LASIK for myopia on corneal aberration.Methods 400 patients who underwent SMILE and femtosecond laser flap LASIK respectively in the refractive clinic of our hospital from October 2018 to August 2019 were selected.The patients who received Smile were in the study group,which was shown as group A.The control group received femtosecond laser assisted LASIK,which was shown as group B.The wavefront aberration analyzer measured the high order aberration,spherical aberration,coma and clover aberration before surgery,1M,3M and 6M after surgery respectively in the dark room environment.Results 1)The aberrations of the two groups were increased after operation,which was statistically different from that before operation.2)Postoperative aberration increase:the aberration increase in SMILE group was lower than that in FS-LASIK group;①There was a statistically significant difference in the amplitude of 1M high order aberration,coma and spherical aberration after surgery(P<0.01).②There was no significant difference in the increase of clover difference after surgery(P>0.05).③The aberration and increase gradually reduce with time.Conclusion New aberrations are introduced by SMILE and FS-LASIK,but the aberrations of the cornea after SMILE increase less.
