摘 要:目的:探讨[牙合]平面与上气道形态功能以及与舌骨位置的相关性及其在错[牙合]畸形患者正畸治疗中的应用。方法:选择2017年12月-2019年6月重庆三峡医药高等专科学校附属医院和银川市口腔医院收治的I类错[牙合]畸形患者181例,根据[牙合]平面角度(OP-SN)将患者分为A组(10°≤OP-SN<15°,n=47)、B组(15°≤OP-SN<20°,n=99)、C组(20°≤OP-SN<25°,n=35)。采用单因素方差分析不同[牙合]平面对上气道形态与舌骨位置的差异比较,分析其相关性。并给予三组患者正畸治疗,比较三组患者上气道形态及舌骨位置。结果:三组患者上气道形态指标OP-SN、SNA、ANB、SNB差异有统计学意义(P<0.05);两两比较,C组患者OP-SN、ANB高于A、B组,且SNA、SNB低于A、B组(P<0.05);三组患者舌骨位置指标JH、H-C3差异有统计学意义(P<0.05);两两比较,C组患者JH、H-C3低于A、B组(P<0.05);OP-SN与SNA、SNB、JH、H-C3呈负相关,与ANB呈正相关(P<0.05);与SPPS、MPS、IPS、S-PNS、Ba-PNS、H-FH、H-PNS、H-MP、H-RGn无显著性差异(P>0.05);治疗后,A组OP-SN、SPPS、H-C3低于治疗前,且SNA、SNB、H-MP高于治疗前(P<0.05);B组OP-SN、SPPS、H-C3、H-PNS低于治疗前,SNA、SNB、Ba-PNS、JH高于治疗前(P<0.05);C组OP-SN、ANB、SPPS、H-C3低于治疗前,SNA、SNB、IPS、S-PNS、Ba-PNS、JH、H-MP高于治疗前(P<0.05);MPS、H-FH、H-PNS、H-RGn无显著性差异(P>0.05)。结论:不同矢状骨面型的[牙合]平面存在差异,而[牙合]平面与上气道形态功能、舌骨位置显著相关,可通过X线机头影测量错[牙合]畸形反映患者病情,在指导治疗中发挥重要意义。Objective To investigate the correlation between occlusal plane and upper airway morphology and hyoid position and its application in orthodontic treatment of patients with malocclusion.Methods A total of 181 patients with type I malocclusion in our hospital from December 2017 to June 2019 were enrolled,and were divided into three groups according to occlusal plane angle(OP-SN),group A(10°≤OP-SN<15°,n=47),group B(15°≤OP-SN<20°,n=99),and group C(20°≤OP-SN<25°,n=35).One-way anova was used to compare the effects of different occlusal planes on upper airway morphology and hyoid position.All patients received orthodontic treatment,and upper airway morphology and hyoid position of three groups were compared.Results The upper airway morphological indicators OP-SN,SNA,ANB,and SNB of three groups had significant difference(P<0.05).Pairwise comparison showed that OP-SN and ANB in group C were higher than those in group A and B,and SNA and SNB were lower than those in group A and B(P<0.05).The hyoid position indicators JH and H-C3 of three groups had significant difference(P<0.05).Pairwise comparison showed that JH and H-C3 in group C were lower than those in group A and B(P<0.05).OP-SN was negatively correlated with SNA,SNB,JH,and H-C3,and was positively correlated with ANB(P<0.05),while had no correlation with SPPS,MPS,IPS,S-PNS,Ba-PNS,H-FH,H-PNS,H-MP and H-RGn(P>0.05).After treatment,the OP-SN,SPPS and H-C3 were decreased,meanwhile,SNA,SNB,and H-MP were increased in group A(P<0.05).After treatment,OP-SN,SPPS,H-C3 and H-PNS were decreased,meanwhile,SNA,SNB,Ba-PNS,and JH were increased(P<0.05).After treatment,OP-SN,ANB,SPPS,and H-C3 were decreased,meanwhile,SNA,SNB,IPS,S-PNS,Ba-PNS,JH,and H-MP were increased(P<0.05).No significant change was found in MPS,H-FH,H-PNS,and H-RGn before and after treatment(P<0.05).Conclusion The occlusal planes of different sagittal bone types are different,and are significantly correlated with upper airway morphology and hyoid position,so the occlusal plane presented by X-ray cepha
