摘　　要：目的:研究计算机辅助设计(Computer aided design,CAD)/计算机辅助制作(Computer aided manufacturing,CAM)氧化锆全瓷冠修复体的美学效果及对牙周组织炎症状态的影响。方法:选择2016年6月-2018年5月笔者医院收治的58例全冠修复患者作为研究对象,随机分为两组。试验组:30例(34颗基牙),给予CAD/CAM氧化锆全瓷冠修复;对照组:28例(32颗基牙),采取镍铬合金烤瓷冠修复。比较两组的龈沟液量、菌斑指数(PI)、牙周袋深度、Furhauser软组织红色美学指数及炎症状态。结果:对照组修复12个月后的龈沟液量、PI、牙周袋深度显著高于试验组,差异均具有统计学意义(P<0.05);对照组修复12个月后的Furhauser软组织红色美学指数总分显著低于试验组,差异具有统计学意义(P<0.05);对照组修复12个月后的IL-6、TNF-α、CRP显著高于试验组,差异均有统计学意义(P<0.05)。结论:CAD/CAM氧化锆全瓷冠修复可显著改善牙周组织损伤及炎症状态,提高全冠修复效果。Objective To study the aesthetic effect of computer aided design/computer aided manufacturing(CAD/CAM)zirconia all-ceramic crown restoration and its effect on state of periodontal tissue inflammation.Methods Fifty-eight patients with full crown repair from June 2016 to May 2018 were selected as subjects.They were randomly divided into the experimental group of 30 patients(34 abutments)given by CAD/CAM zirconia all-ceramic crown repair,and the control group of 28 example(32 abutments)treated with repaired with a nickel-chromium alloy porcelain crown.Gingival crevicular fluid volume,plaque index(PI),periodontal pocket depth,Furhauser soft tissue red aesthetic index,and inflammatory status were compared between the two groups.Results The gingival crevicular fluid volume,PI,and periodontal pocket depth of the control group after 12 months of repair were significantly higher than the experimental group,and the difference was statistically significant(P<0.05).The total score of Furhauser soft tissue red aesthetic index after 12 months of repair in the control group was significantly lower than that in the experimental group,and the difference was statistically significant(P<0.05).IL-6,TNF-αand CRP in the control group after 12 months of repair were significantly higher than those in the experimental group(P<0.05).Conclusion CAD/CAM zirconia all-ceramic crown repair can significantly improve periodontal tissue damage and inflammation,and improve the effect of full crown restoration.
