石英纤维带粘接自体牙冠结合截根术在过渡性牙周夹板中的应用

摘　　要：目的:对牙周炎Ⅲ度松动前牙,用石英纤维带行松牙固定配合截根术,评价此夹板粘接自体牙冠的临床存活率。方法:选择38例有1~4颗Ⅲ度松动前牙且邻牙牙周支持组织丧失严重、不符合种植义齿及全冠固定修复的牙周炎患者行松牙固定,截除Ⅲ度松动牙牙根,评价1年内固定效果,记录不同时段邻牙附着丧失(AL)、探诊出血(BOP)阳性率,以配对t检验分析数据。结果:术后3、6、12个月固定有效率为100.00%、97.37%、92.11%;邻牙AL随固定时间延长逐渐减小,差异有统计学意义(P<0.001);BOP阳性率由治疗前的(74.32±9.93)%减至治疗后1个月的(32.05±5.20)%,差异有统计学意义(P<0.001);比较固定后各时段的BOP阳性率无显著性差异。结论:对Ⅲ度松动前牙伴邻牙牙周支持组织受损的Ⅲ~Ⅳ期牙周炎患者,行松牙固定配合截根术,以纤维条粘接存留牙冠行过渡修复,在1年期内固定效果良好,且改善邻牙牙周状况。Objective To evaluate the clinical survival rate of the splint in the treatment of periodontitis withⅢdegree loosening of anterior teeth and the fixation of loose teeth with quartz fiber tape combined with root cutting.Methods 38 patients with periodontitis with 1~4Ⅲdegree loose anterior teeth and severe periodontal support tissue loss of adjacent teeth,which did not conform to implant denture and full crown fixed repair,were selected to remove theⅢdegree loose teeth roots,evaluated the effect of fixation within one year,recorded the positive rate of attachment loss(AL)and bleeding(BOP)of adjacent teeth in different periods,and match the t-test analysis data.Results The fixed effective rates were 100.00%,97.37%,and 92.11%at the 3rd,6th,and 12th months after operation.The AL of adjacent teeth gradually decreased with the extension of the fixed time,and the difference was statistically significant(P<0.001).The BOP positive rate decreased from(74.32±9.93)%before treatment to(32.05±5.20)%one month after treatment,and the difference was significant(P<0.001),there was no statistical difference in the BOP positive rate in each period after the fixation.Conclusion Patients with stageⅢ~Ⅳperiodontitis with loosening of the anterior teethⅢand accompanied by damage to adjacent periodontal support tissues are given loose tooth fixation and root cutting,and the remaining crowns are bonded with fiber strips for transitional repair.The treatment method has a good fixation effect within one year,and improves the periodontal condition of adjacent teeth.
