摘  要:目的:探究宫颈上皮瘤病变患者采用宫颈锥切术对其妊娠结局的影响。方法:选取2015-2019年在国内外数据库中发表的关于宫颈锥切术对妊娠结局影响的随机对照试验文献资料进行计算机检索,并利用纽卡斯尔-渥太华量表(NOS)对检索的文献资料进行质量评价,随后进行Meta分析。结果:本次纳入文献资料9篇,主要包括有宫颈电圈环切术(LEEP)组和宫颈冷刀锥切入(CKC)组,以上两组与各自对照组比较,其胎膜早破发生率、胎儿生长受限发生率、早产率均较高,差异有统计学意义(P<0.05);虽然LEEP组和CKC组流产率比各自对照组高,但差异无统计学意义(P>0.05);LEEP组和CKC组剖宫产率与各自对照组比较,差异无统计学意义(P>0.05)。结论:宫颈锥切术不会增加流产率和剖宫产率,但是会对术后早产、新生儿生长受限以及胎膜早破产生影响,增加其发生率。Objective:To explore the effect of cervical conization on pregnancy outcome in patients with cervical epithelioma lesions.Methods:The literature of randomized controlled trials on the impact of cervical conization on pregnancy outcomes published in domestic and foreign databases from 2015 to 2019 were selected for computer retrieval,the Newcastle-Ottawa Scale(NOS)was used to evaluate the quality of the retrieved literature,followed by Meta analysis.Results:There were 9 references included in this study,mainly including LEEP group and CKC group,the above two groups were compared with their respective control groups,the incidence of premature rupture of membranes,fetal growth restriction and premature birth were all higher,with the differences were statistically significant(P<0.05).Although abortion rates were higher in the LEEP and CKC groups than in the control groups,but the difference was not statistically significant(P>0.05).The cesarean section rate of LEEP group and CKC group was compared with the control group,but the difference was not statistically significant(P>0.05).Conclusions:Cervical conization does not increase the rate of miscarriage or cesarean section,however,it has an effect on postoperative preterm birth,neonatal growth restriction and premature rupture of membranes,increasing their incidence.
