D-二聚体增高的恶性肿瘤患者预防性抗凝治疗临床效果及安全性研究

摘　　要：目的:探讨预防性抗凝治疗D-二聚体增高恶性肿瘤患者的效果及安全性。方法:2018年1月-5月收治恶性肿瘤患者163例,检测发现D-二聚体水平增高74例;随机分为抗凝组35例和未抗凝组39例。未抗凝组进行常规化疗及对症治疗;抗凝组在常规化疗及支持治疗的同时给予预防性抗凝治疗。比较两组患者治疗后静脉血栓栓塞症(VTE)发生率、不良反应发生率及D-二聚体水平变化情况。结果:抗凝组VTE发生率低于未抗凝组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05);抗凝组治疗后D-二聚体水平较治疗前明显降低,差异有统计学意义(P<0.05)。结论:预防性抗凝治疗有助于改善D-二聚体水平,降低恶性肿瘤VTE发生率,且不增加临床出血的风险。Objective:To explore the effect and safety of preventive anticoagulant therapy on malignant tumor patients with increased D-dimer.Methods:From January to May 2018,163 cases of patients with malignant tumor were enrolled,74 cases of D-dimer level found increased,they were randomly divided into non-anticoagulant group(n=39)and anticoagulant group(n=35).The non-anticoagulant group was treated with conventional chemotherapy and symptomatic treatment,while the anticoagulant group was given preventive anticoagulant therapy with conventional chemotherapy and supportive therapy.The therapeutic effects between the two groups was compared.Results:The incidence of VTE in anticoagulant group was lower than that in non-anticoagulant group,the difference was statistically significant(P<0.05).The incidence of adverse reaction between the two groups was compared,the difference was not statistically significant(P>0.05).After treatment,D-dimer level in anticoagulant group was significantly lower than before treatment,the difference was statistically significant(P<0.05).Conclusion:Prophylactic anticoagulant therapy can improve the level of D-dimer,reduce the incidence of VTE in malignant tumors,it do not increase the risk of bleeding in clinical.
