摘 要:目的观察幼年大鼠反复热性惊厥对远期癫痫发作及海马突触素(SYN)表达的影响,探讨反复热性惊厥影响癫痫易感性的可能机制。方法45只21日龄雄性SD大鼠随机分为热性惊厥组、发热对照组和正常对照组,每组15只。利用热水浴诱导热性惊厥组大鼠发生10次热性惊厥,间隔2月后匹罗卡品10 mg/kg腹腔注射诱发热性惊厥组、发热对照组和正常对照组大鼠癫痫持续状态,观察出现癫痫自发反复发作(spontaneous recurrent seizures,SRS)间期及SRS频率。采用免疫组化染色及蛋白免疫印迹法观察大鼠海马SYN的表达。结果热性惊厥组、发热对照组和正常对照组出现SRS间期分别为(10.95±1.72)d、(15.58±7.23)d和(16.29±5.81)d;每周SRS次数分别为(10.56±2.32)次、(7.17±1.42)次和(6.65±1.24)次。与发热对照组和正常对照组比较,热性惊厥组SRS间期缩短(P<0.05),每周SRS次数增加(P<0.05)。大鼠海马SYN免疫组化阳性染色为棕黄色点状或颗粒状沉积,呈明显免疫标记板层分布,细胞层不染色,分子层和多形层染色较深。与发热对照组、正常对照组比较,热性惊厥组海马SYN蛋白表达量明显升高(P<0.05);发热对照组和正常对照组海马SYN蛋白表达量比较差异无统计学意义(P>0.05)。结论幼年大鼠反复热性惊厥使得大鼠远期的癫痫易感性增加,可能与海马SYN表达升高有关。Objective To observe effects of febrile seizures on long-term epilepsy susceptibility and hippocampal synaptophysin(SYN)in expression in immature rats,and to explore the possible mechanism of recurrent febrile seizure affecting epilepsy susceptibility.Methods 4521-day-old male SD rats were randomly divided into three groups:febrile seizure group,febrile control group and normal control group,with 15 rats in each group.Rats in the febrile seizure group were induced by hot water bath for 10 times.After 2 months,the epileptic status of rats in the control group,the fever control group and the normal control group were induced by intraperitoneal injection of 10 mg/kg pilocarpine.Spontaneous and recurrent episodes(SRS)and SRS frequencies were observed.Immunohistochemical staining and western blotting were used to observe the expression of SYN in rat hippocampus.Results The SRS intervals in febrile seizure group,febrile control group and normal control group were(10.95±1.72)days,(15.58±7.23)days and(16.29±5.81)days,respectively,and the SRS frequencies per week were(10.56±2.32)times,(7.17±1.42)times and(6.65±1.24)times,respectively.Compared with febrile control group and normal control group,SRS interval in febrile seizure group was shorter(P<0.05),and SRS frequency per week was increased(P<0.05).The positive staining of SYN in the hippocampus was brownish yellow spots or granules,with obvious distribution of immunomarker plate layer,no staining of cell layer,and deep staining of molecular layer and polymorphic layer.The expression of SYN in hippocampus of febrile seizure group was significantly increased(P<0.05),and there was no significant difference between the febrile control group and normal control group(P>0.05).Conclusion Febrile seizures in immature rats can increase the long-term epilepsy susceptibility,which may related to the increased expression of SYN in hippocampus.
