摘 要:目的研究负性共刺激分子T淋巴细胞免疫球蛋白黏蛋白分子-3(TIM-3)在口腔鳞状细胞癌(OSCC)患者外周血、癌组织及癌旁组织中CD4^+T、CD8^+T淋巴细胞上的表达及其临床意义。方法选取2018年4月至2019年10月郑州大学第一附属医院收治的41例OSCC患者作为OSCC组,选取同期20例健康体检者作为对照组。通过流式细胞术检测CD4^+T、CD8^+T淋巴细胞上TIM-3的表达水平,进一步探讨其与OSCC患者临床病理特征的关系。结果TIM-3在OSCC患者外周血CD4^+T、CD8^+T淋巴细胞上的表达水平高于对照组(P<0.05)。TIM-3在OSCC患者癌组织中CD4^+T、CD8^+T淋巴细胞上的表达水平高于癌旁组织(P<0.05)。有淋巴结转移的OSCC患者外周血中CD4^+T、CD8^+T淋巴细胞上TIM-3的表达水平高于无淋巴结转移的OSCC患者(P<0.05)。TIM-3在OSCC患者癌组织中CD4^+T、CD8^+T淋巴细胞上的表达水平与年龄、性别、部位、肿瘤大小、病理学分级、TNM分期、淋巴结转移无关(P>0.05)。结论TIM-3在OSCC患者外周血和癌组织中CD4^+T、CD8^+T淋巴细胞上呈高表达,且OSCC患者外周血中CD4^+T、CD8^+T淋巴细胞上TIM-3的表达水平与淋巴结转移有关,TIM-3可能参与OSCC的发生发展,并有望成为OSCC新的生物标志物及免疫治疗的新靶点。Objective To investigate the expression and clinical significance of negative costimulatory molecule T-cell immunoglobulin mucin-3(TIM-3)on CD4^+T lymphocytes and CD8^+T lymphocytes in peripheral blood,tumor tissues and paracancerous tissues of patients with oral squamous cell carcinoma(OSCC).Methods A total of 41 patients with OSCC who were admitted to the First Affiliated Hospital of Zhengzhou University from April 2018 to October 2019 were selected as OSCC group,and at the same period 20 healthy people were selected as control group.Flow cytometry was used to detect the expression levels of TIM-3 on CD4^+T lymphocytes and CD8^+T lymphocytes,and the relationship between TIM-3 and clinicopathological features of patients with OSCC was further explored.Results The expression levels of TIM-3 on CD4^+T lymphocytes and CD8^+T lymphocytes in peripheral blood of patients with OSCC were higher than those in control group(P<0.05),and the expression levels of TIM-3 on CD4^+T lymphocytes and CD8^+T lymphocytes in tumor tissues of patients with OSCC were higher than those in paracancerous tissues(P<0.05).The expression levels of TIM-3 on CD4^+T lymphocytes and CD8^+T lymphocytes in peripheral blood of patients with OSCC with lymph node metastasis were higher than those in patients with OSCC without lymph node metastasis(P<0.05).The expression levels of TIM-3 on CD4^+T lymphocytes and CD8^+T lymphocytes in tumor tissues of patients with OSCC were not correlated with age,sex,location,tumor size,pathological grade,TNM stage and lymph node metastasis(P>0.05).Conclusion TIM-3 is highly expressed on CD4^+T lymphocytes and CD8^+T lymphocytes in peripheral blood and tumor tissues of patients with OSCC.The expression levels of TIM-3 on CD4^+T lymphocytes and CD8^+T lymphocytes in peripheral blood of patients with OSCC are related to lymph node metastasis.TIM-3 may be involved in the occurrence and development of OSCC,and it is expected to be a new biomarker and a new target of immunotherapy of OSCC.
