超声穿刺引导在重症胰腺炎引流中的应用研究

摘　　要：目的:研究超声穿刺引导在重症胰腺炎引流中的应用。方法:选取2018年4月-2019年4月新乡医学院第一附属医院收治的重症胰腺炎患者82例,按照随机数表法分为对照组和观察组,每组各41例。对照组接受保守治疗,观察组在此基础上给予超声穿刺引导引流治疗。对比两组体温正常时间、白细胞恢复正常时间、血淀粉酶恢复正常时间、并发症发生率以及病死率,观察两组治疗前后的急性生理学及慢性健康状况评分系统(APACHE-Ⅱ)评分。结果:观察组体温正常时间、白细胞恢复正常时间、血淀粉酶恢复正常时间均明显较对照组短(P<0.05);两组治疗前的APACHEⅡ评分对比差异无统计学意义(P>0.05);观察组治疗后的APACHEⅡ评分明显较对照组低(P<0.05);与对照组相比,观察组的并发症总发生率较低(P<0.05);两组病死率对比差异无统计学意义(P>0.05)。结论:超声穿刺引导应用于重症胰腺炎患者的引流中能够缩短患者症状改善时间,且可以降低并发症以及病死率。Objective:To study the application of ultrasound guidance in the drainage of severe pancreatitis.Methods:82 patients with severe pancreatitis in the hospital from April,2018 to April,2019 were selected and divided into two groups according to the random number table method,with 41 cases in each group.The control group received conservative treatment,and the observation group received ultrasound guided drainage on this basis.The acute physiological and chronic health(APACHE-II)scores before and after treatment were observed.Results:The time of temperature,leukocyte and amylase in the observation group were significantly shorter than those in the control group(P<0.05).There was no significant difference between the two groups in the Apache I score before treatment(P>0.05).The Apache II score in the observation group after treatment was significantly lower than that in the control group(P<0.05).Compared with the control group,the total incidence of complications in the observation group was lower(P<<0.05).There was no significant difference in mortality between the two groups(P>0.05).Conclusion:Utrasound guided drainage can shorten the time of symptom improvement,reduce the complications and mortality.
