附子理中汤加味对溃疡性结肠炎患者肠壁组织及全身炎症水平的改善作用

摘　　要：目的从结肠镜组织学检查和炎症因子角度探讨附子理中汤加味治疗溃疡性结肠炎(UC)的效果。方法选择2019年1—12月盘锦市中医医院收治的UC患者74例,随机分为对照组和观察组各37例。两组均采用柳氮磺砒啶片进行治疗,观察组在此基础上加用附子理中汤加味。比较两组治疗前后的中医证候积分、结肠镜组织学和炎症因子改善情况。结果经治疗后,观察组腹痛、腹泻、里急后重、脓血便和证候总分均低于对照组(P<0.05);观察组结肠镜组织学分级优于对照组(P<0.05);观察组IL-6、IL-8、TNF-α、MMP-2和MMP-9水平均低于对照组(P<0.05)。结论附子理中汤加味可以进一步改善UC患者的临床症状、肠壁组织状态和全身炎症水平,值得临床推广应用。Objective To investigate the effect of Fuzi Lizhong Decoction on ulcerative colitis(UC)from the perspective of colonoscopy histological examination and inflammatory factors.Methods A total of 74 patients with UC in Panjin Hospital of traditional Chinese medicine from January to December 2019 were randomly divided into control group and observation group,37 cases in each group.Both groups were treated with sulfasalazine tablets,and the observation group was treated with Fuzi Lizhong decoction on the basis of sulfasalazine tablets.The traditional Chinese medical syndrome score,colonoscopy histology and inflammatory factors were compared between the two groups before and after treatment.Results After treatment,the total scores of abdominal pain,diarrhea,tenesmus,bloody stool and syndrome in the observation group were significantly lower than those in the control group(P<0.05).The histological grade of colonoscopy in the observation group was significantly better than that in the control group(P<0.05).Moreover,the levels of IL-6,IL-8,TNF-α,MMP-2 and MMP-9 in the observation group were significantly lower than those in the control group(P<0.05).Conclusion The modified Fuzi Lizhong decoction can further improve the clinical symptoms,intestinal wall tissue status and systemic inflammation level of UC patients,which is worthy of clinical application.
