摘　　要：眼瘢痕性类天疱疮(OCP)是一种累及眼部的慢性瘢痕化的自身免疫性疾病,呈慢性、进展性过程,若不及时治疗导致眼表结构受损可致盲。OCP的诊断主要依靠组织活检证明免疫球蛋白或补体沉积,或直接免疫荧光(DIF)检查显示免疫球蛋白或补体在基底膜区呈线性沉积。DIF阴性不能排除对OCP的诊断,若患者的瘢痕形成活跃或呈进行性,则应行全身免疫抑制治疗。OCP的治疗主要是通过全身免疫抑制控制炎症,局部药物治疗往往无效。在全身炎症控制稳定的情况下,可通过手术改善眼表结构和功能。OCP诊治的关键在于早发现、早治疗。Ocular cicatricial pemphigoid(OCP)is a chronic scarring autoimmune disease that affects the eye,which is a chronic and progressive process.If it is not treated in time,the ocular surface structure will eventually be damaged and cause blindness.The diagnosis of OCP mainly relies on tissue biopsy to prove the deposition of immunoglobulin or complement,or direct immunofluorescence(DIF)examination shows that immunoglobulin protein or complement is linearly deposited in the basement membrane area.A negative DIF result cannot exclude the diagnosis of OCP.If the patient′s scar formation is active or progressive,systemic immunosuppressive therapy should be given.The treatment of OCP is mainly to control inflammation through systemic immunosuppression,and local drug treatment is often ineffective.Under stable control of systemic inflammation,operations can be used to improve the structure and function of the ocular surface.The key to OCP diagnosis and treatment is early detection and early treatment.
