摘 要:目的探讨Piwil2蛋白和细胞周期蛋白D1(CyclinD1蛋白)在乳腺恶性叶状肿瘤组织中的表达及其相关性。方法收集1990年1月至2015年2月陆军第八十一集团军医院病理科和解放军总医院第七医学中心病理科存档的60例女性乳腺恶性叶状肿瘤患者的资料进行回顾性分析,同时选取60例正常乳腺组织作为对照组。采用免疫组织化学(EnVision TM)法分别检测乳腺恶性叶状肿瘤组织和正常乳腺组织中Piwil2蛋白和CyclinD1蛋白的阳性表达情况,分析其表达与乳腺恶性叶状肿瘤相关临床病理因素之间的关系以及Piwil2蛋白和CyclinD1蛋白的相关性。结果Piwil2蛋白和CyclinD1蛋白在乳腺恶性叶状肿瘤组织中的阳性表达率分别为88.33%(53/60)、76.67%(46/60),但在正常乳腺组织均未表达。肿瘤直径≥9 cm、Ki-67≥25%、瘤细胞异型显著以及核分裂象≥15个/10个高倍镜视野的患者Piwil2蛋白表达水平高于肿瘤直径<9 cm、Ki-67<25%、瘤细胞异型不显著以及核分裂象<15个/10个高倍镜视野者(P<0.05)。Ki-67≥25%、瘤细胞异型性显著及核分裂象≥15个/10个高倍镜视野者CyclinD1蛋白的表达水平高于Ki-67<25%、瘤细胞异型不显著以及核分裂象<15个/10个高倍镜视野者(P<0.05)。乳腺恶性叶状肿瘤组织中Piwil2蛋白的表达与CyclinD1蛋白的表达呈正相关(r=0.413,P<0.01)。结论Piwil2蛋白和CyclinD1蛋白的过表达参与了乳腺恶性叶状肿瘤的形成,其表达水平可作为评判瘤细胞分裂、增殖的参考指标。Objective To investigate the expression of P-element-induced wimpy testis like 2(Piwil2)protein and CyclinD1 protein in breast malignant phyllodes tumor tissue and study their correlations.Methods The malignant phyllodes tumor tissue of 60 cases were collected in the Department of Pathology of PLA 81st Group Military Hospital and the Department of Pathology of the Seventh Medical Center of PLA General Hospital from Jan.1990 to Feb.2015,and 60 cases of normal breast tissue were included as a control group.The positive expression of Piwil2 protein and CyclinD1 protein was respectively detected by immunohistochemisty(Envision TM)in malignant phyllodes tumor tissue and normal breast tissue,and the relationship between the expression with the pathological parameters and the correlation between Piwil2 protein and CyclinD1 protein were analyzed.Results The positive expression rate of Piwil2 protein and CyclinD1 protein in breast malignant phyllodes tumor tissue were 88.33%(53/60)and 76.67%(46/60),respectively,while there was no expression in normal breast tissue.The expression levels of Piwil2 protein in the patients with tumor diameter≥9 cm,Ki-67≥25%,oncocyte atypia significance and Karyokinesis≥15/10 HPF were higher than the patients with tumor diameter<9 cm,Ki-67<25%,oncocyte atypia insignificance and Karyokinesis<15/10 HPF(P<0.05).The expression levels of CyclinD1 protein in the patients with Ki-67≥25%,oncocyte atypia significance and Karyokinesis≥15/10 HPF were higher than the patients with Ki-67<25%,oncocyte atypia insignificance and Karyokinesis<15/10 HPF(P<0.05).The expression of Piwil2 protein and CyclinD1 protein was positively correlated(r=0.413,P<0.01).Conclusion The overexpression of Piwil2 protein and CyclinD1 protein is associated with the development of breast malignant phyllodes tumor,and their expression levels can be used as reference for judging tumor cell cleavage and proliferation.
