甲地孕酮联合TC化疗方案治疗晚期卵巢上皮性癌患者的疗效评价

摘　　要：目的探讨甲地孕酮联合紫杉醇+卡铂(TC)化疗方案治疗晚期卵巢上皮性癌患者的临床疗效。方法选取晚期卵巢上皮性癌患者80例(2016年1月~2019年12月),根据随机数字表分为研究组(n=40)、常规组(n=40)。常规组采用TC化疗方案,研究组于常规组基础上加用甲地孕酮治疗,统计比较两组临床疗效,治疗前、治疗4个周期后血清DEAD家族多肽4(DDX4)、血管内皮生长因子(VEGF)、基质金属蛋白酶2(MMP-2)水平及不良反应发生率。结果研究组疾病控制率(DCR)75.00%高于常规组52.50%(P<0.05);治疗4个周期后研究组血清VEGF、MMP-2、DDX4水平低于常规组(P<0.05);研究组恶心呕吐(5.00%)、腹泻腹痛(5.00%)发生率低于常规组的25.00%、22.50%(P<0.05)。结论甲地孕酮联合TC化疗方案治疗晚期卵巢上皮性癌患者疗效显著,可降低其血清VEGF、MMP-2、DDX4水平,且安全性高。Objective To investigate the clinical efficacy of megestrol combined with Taxol and Carboplatin(TC)chemotherapy in the treatment of patients with advanced epithelial ovarian cancer.Methods A total of 80 patients with advanced epithelial ovarian cancer from January 2016 to December 2019 were selected and divided into study group(n=40)and conventional group(n=40)by random number table method.The conventional group were treated with TC chemotherapy,and the study group were treated with megestrol on the basis of the conventional group.And then,the clinical efficacy,the serum levels of DEAD family polypeptide 4(DDX4),vascular endothelial growth factor(VEGF)and matrix metalloproteinase-2(MMP-2)before treatment and 4 cycles of treatment,and the incidence of adverse reactions were statistically compared between the two groups.Results The disease control rate(DCR)of the study group was 75.00%,which was higher than that(52.50%)of the conventional group(P<0.05).After 4 cycles of treatment,the serum levels of VEGF,MMP-2 and DDX4 in the study group were lower than those in the conventional group(P<0.05).The incidence of nausea and vomiting(5.00%),diarrhea and abdominal pain(5.00%)in the study group was lower than that in the conventional group(22.50%,22.50%,P<0.05).Conclusion Megestrol combined with TC chemotherapy has a significant effect on patients with advanced epithelial ovarian cancer,which can reduce the serum levels of VEGF,MMP-2 and DDX4 with high safety.
