阴道放置卡孕栓及米索前列醇宫颈预处理对宫腔镜Ⅰ型子宫黏膜下肌瘤电切术疗效的影响分析

摘　　要：目的观察宫腔镜Ⅰ型子宫黏膜下肌瘤电切术前实施阴道放置卡孕栓及米索前列醇宫颈预处理的效果。方法选取2017年2月至2019年2月本院收治90例Ⅰ型子宫黏膜下肌瘤行宫腔镜肌瘤电切术治疗的患者,随机分为观察组与对照组,各45例。对照组采用阴道放置米索前列醇进行宫颈预处理,观察组采用阴道放置卡孕栓进行宫颈预处理,比较两组宫颈扩张效果。结果观察组宫颈扩张宽度为(7.97±0.89)mm,大于对照组的(6.24±0.55)mm,差异有统计学意义(P<0.05);观察组宫颈扩张有效率为95.56%,高于对照组的80.00%,差异有统计学意义(P<0.05);观察组手术时间为(27.58±8.43)min,少于对照组的(39.94±10.16)min,差异有统计学意义(P<0.05);观察组术中出血量为(21.82±9.77)mL,少于对照组的(40.39±13.07)mL,差异有统计学意义(P<0.05);观察组术后不良反应发生率为13.33%,对照组为11.11%,差异无统计学意义。结论在对Ⅰ型子宫黏膜下肌瘤患者实施宫腔镜电切术治疗前,采用阴道放置卡孕栓进行宫颈预处理,具有较好的宫颈扩张、软化效果,可缩短手术时间,减少术中出血量,且术后不良反应较少,有利于患者术后尽早恢复,值得临床推广应用。Objective To observe the effect of vaginal carboprost suppository and misoprostol cervical pretreatment before hysteroscopic submucosal myoma resection.Methods From February 2017 to February 2019,90 patients with typeⅠsubmucous myoma underwent hysteroscopic myomectomy in our hospital were randomly divided into observation group and control group,with 45 cases in each group.The control group was treated with misoprostol through vagina,while the observation group was treated with carboprost suppository.Compare the effects of cervical dilation between the two groups.Results The cervical dilatation width of the observation group was(7.97±0.89)mm,which was larger than(6.24±0.55)mm of the control group,the difference was statistically significant(P<0.05).The effective rate of cervical dilatation in the observation group was 95.56%,higher than 80.00%in the control group,the difference was statistically significant(P<0.05).The operation time of the observation group was(27.58±8.43)min,which was less than(39.94±10.16)min in the control group,the difference was statistically significant(P<0.05).The intraoperative blood loss in the observation group was(21.82±9.77)mL,which was less than(40.39±13.07)mL in the control group,the difference was statistically significant(P<0.05).The incidence of adverse reactions in the observation group was 13.33%,which was 11.11%in the control group,and the difference was not statistically significant.Conclusion In the treatment of typeⅠsubmucous myoma patients with hysteroscopic resection,the use of vaginal carboprost suppository for cervical pretreatment,has a good effect of cervical dilatation and softening,can reduce the operation time and intraoperative blood loss,and less adverse reactions,which is conducive to the early recovery of patients after surgery,it is worthy of clinical application.
