摘　　要：目的:探讨胃癌组织中尾型同源盒转录因子2(CDX2)、miRNA-32表达与患者临床病理参数、预后的关系。方法:收集50例胃癌组织,采用免疫组化检测CDX2蛋白表达,分析CDX2表达与患者临床病理参数及预后的关系;实时荧光定量PCR检测肿瘤和癌旁组织中CDX2 mRNA和miRNA-32表达,分析两者相关性及其与临床病理参数的关系。结果:胃癌组织中CDX2蛋白阳性率为58.0%,CDX2蛋白阳性表达率淋巴结转移患者为45.2%,临床Ⅲ~Ⅳ期患者为46.9%,分别低于无淋巴结转移患者的78.9%和临床Ⅰ~Ⅱ期患者的77.8%,差异均有统计学意义(P<0.05)。与CDX2阳性患者比较,CDX2阴性患者术后2年和3年累计复发率分别为80.9%和95.2%,高于CDX2阳性患者的44.8%和69.0%,差异均有统计学意义(P<0.05),中位复发时间和5年生存率低于CDX2阳性患者,差异均具有统计学意义(P<0.05)。CDX2 mRNA和miRNA-32表达与肿瘤浸润深度、临床分期、淋巴结转移有关,差异均有统计学意义(P<0.05),miRNA-32与CDX2 mRNA表达呈正相关(r=0.861,P<0.05)。结论:胃癌组织CDX2和miRNA-32表达下调或缺失与患者临床病情进展相关,术后易复发,生存期缩短,预后不良。Objective:To investigate the expression of CDX2 and miRNA-32 in gastric cancer(GC)and its relationship with clinicopathological parameters and prognosis for patients.Methods:The expression of CDX2 in 50 GC cases was detected by immunohistochemistry,and the survival data of patients were followed up.The clinical significance of CDX2 expression with clinicopathological parameters,postoperative recurrence and survival time was analyzed;the expression of CDX2 mRNA and miRNA-32 in tumor and adjacent tissues was detected by RT-PCR,and the correlation and clinical significance between them were analyzed.Results:The positive rate of CDX2 protein in GC was 58.0%.The positive expression of CDX2 protein was related to TNM stage and lymph node metastasis(P<0.05).The median recurrence time and survival time of patients with CDX2 positive expression were significantly longer than those with CDX2 negative expression(all P<0.05).The expressions of CDX2 mRNA and miRNA-32 were correlated with tumor invasion depth,TNM stage and lymph node metastasis(P<0.05).The correlation analysis showed that the expression of miRNA-32 and CDX2 mRNA was positively correlated(r=0.861,P<0.05).Conclusions:The down-regulation or deletion of CDX2 and miRNA-32 expression in GC tissues was related to the clinical progress of patients,which was easy to relapse early after operation and shorten the survival period.
