带中央孔的ICL植入矫正中高度近视的效果

摘　　要：目的评价有晶状体眼后房型人工晶状体(implantable collamer lens,ICL)V4c型(带中央孔)植入矫正中高度近视的效果.方法回顾性研究.分析2015年7月至2018年12月山西省眼科医院中高度近视60例(118眼)的临床资料.患眼屈光状态为等效球镜度(SE)(-11.26±3.87)D.所有患者术中植入均带中央孔的V4c型ICL,随访12个月,观察术后各项指标.结果随访期末,92.7%(76/82)的术眼实际屈光状态与预期屈光状态(SE)的差值在±1.0 D以内,差值在±0.5 D以内占88.6%(78/88);手术前后眼压差异无统计学意义(F=0.850,P>0.05);角膜内皮细胞累计丢失率为0.14%~5.90%,平均为1.44%±3.25%.术后1天,1、3、6及12个月拱高分别为:(0.635±0.201)mm、(0.608±0.194)mm、(0.581±0.195)mm、(0.597±0.171)mm及(0.495±0.158)mm.患者满意程度为100%.结论V4c型ICL矫正中高度近视具有良好的有效性、安全性、可预测性及稳定性.Objective To evaluate the efficacy of the implantable collamer lens(ICL)V4c with a central hole to correct moderate and high myopia.Methods This was a retrospective study.The clinical data of 118 eyes of 60 cases with moderate and high myopia from Jul.2015 to Dec.2018 in Shanxi Eye Hospital were analysed.All cases were implanted in the posterior chamber with the V4c ICL with a central hole.The refractive status,spherical equivalent was-3.25 D to-20.63 D,with an average of(-11.26±3.87)D.The postoperative indicators were analysed.Results At the end of follow-up time,the eyes which had a difference between the actual refractive status and the desired refractive status within±1.0 D were 92.7%(76/82)and 88.6%(78/88)of eyes within±0.5 D.The difference in IOP was not statistically significant before and after operation(F=0.850,P>0.05).The average loss rate of corneal endothelium cells was 1.44%±3.25%(0.14%-5.90%).The vaults of ICL were(0.635±0.201)mm,(0.608±0.194)mm,(0.581±0.195)mm,(0.597±0.171)mm,(0.495±0.158)mm at 1 day,1 month,3 months,6 months,and 12 months after surgery.The degree of satisfaction of patients was 100%.Conclusion The V4c ICL is effective,safe,predictable and stable to correct moderate and high myopia.
