摘 要:目的探讨自我管理教育对乳腺癌经外周静脉穿刺中心静脉置管(PICC)化疗患者自我管理能力、化疗依从性及并发症的影响。方法根据干预方式的不同将85例乳腺癌PICC化疗患者分为观察组(n=44)和对照组(n=41),对照组患者采用常规管理模式,观察组患者在对照组的基础上进行自我管理教育。比较干预前及干预1个月后两组患者的自我管理能力、化疗依从性及并发症发生情况。结果干预后,两组患者维护信息获取、维护依从性、日常导管监测、异常情况处理、运动管理、日常生活管理、自我管理信心评分及总分均高于本组干预前,且观察组患者的上述评分均高于对照组,差异均有统计学意义(P﹤0.05)。干预后,观察组中年龄﹤45岁乳腺癌患者的自我管理能力评分明显高于年龄≥45岁的患者,差异有统计学意义(P﹤0.01)。干预后,观察组患者的化疗依从率为93.18%,高于对照组的82.93%,差异有统计学意义(P﹤0.05)。观察组患者的并发症总发生率为9.09%,低于对照组的29.27%,差异有统计学意义(P﹤0.05)。结论自我管理教育可提高乳腺癌PICC化疗患者的自我管理能力,改善其化疗依从性,降低相关并发症发生率。Objective To explore the effect of self-management education on self-management ability,chemotherapy compliance and complications of breast cancer patients undergoing peripherally inserted central venous catheter(PICC)chemotherapy.Method 85 breast cancer patients treated with PICC chemotherapy were divided into the control group(n=41,treated with routine management)and observation group(n=44,treated with self-management education on the basis of control group).The scores of self-management ability,chemotherapy compliance and complications before and 1 month after intervention were compared between the two groups.Result After intervention,the scores of maintenance information acquisition,maintenance compliance,daily catheter monitoring,abnormal situation management,exercise management,daily life management,self-management confidence and total scores of the two groups were significantly higher than those before intervention,with higher scores in the observation group than those in the control group(P<0.05).After intervention,the scores of self management ability for patients<45 years in the observation group were significantly higher than that for patients≥45 years(P<0.01).After intervention,the chemotherapy compliance rate was significantly higher in the observation group than that in the control group(93.18%vs 82.93%)(P<0.05).The incidence of complications was significantly lower in the observation group than that in the control group(9.09%vs 29.27%)(P<0.05).Conclusion Self-management education can improve the self-management ability of breast cancer patients undergoing PICC chemotherapy,improve chemotherapy compliance and reduce the incidence of complications.
