低能量激光照射联合曲安奈德软膏局部涂擦及碳酸氢钠含漱治疗牙龈糜烂型扁平苔藓的研究

摘　　要：目的研究低能量激光照射联合曲安奈德软膏局部涂擦及碳酸氢钠含漱治疗牙龈糜烂型扁平苔藓(LP)的效果。方法选取2014年12月~2019年12月收治的牙龈糜烂型LP患者80例,按照随机数字表法分为对照组、观察组,各40例。对照组采用曲安奈德软膏局部涂擦及碳酸氢钠含漱治疗,观察组在对照组基础上采用低能量激光照射治疗。比较两组疗效、治疗前、治疗1个月后糜烂面积、疼痛程度[视觉模拟量表(VAS评分)]及唾液中分泌性免疫球蛋白A(IgA)、白蛋白(ALB)水平。结果观察组总有效率92.50%,高于对照组75.00%,差异有统计学意义(P<0.05);治疗1个月后观察组糜烂面积、VAS评分小于对照组,差异有统计学意义(P<0.05);治疗1个月后观察组唾液IgA水平高于对照组,ALB水平低于对照组,差异有统计学意义(P<0.05)。结论低能量激光照射联合曲安奈德软膏局部涂擦及碳酸氢钠含漱治疗牙龈糜烂型LP疗效显著,能缩小糜烂面积,减轻疼痛程度,改善唾液中IgA、ALB水平。Objective To study the effect of low energy laser irradiation combined with triamcinolone ointment local rubbing and sodium bicarbonate gargle on gingival erosive lichen planus(LP).Methods Eighty patients with gum erosion type LP in our hospital(December 2014 to December 2019)were selected and divided into a control group and an observation group,each with 40 cases according to the random number table method.The control group was treated with triamcinolone ointment topical rubbing and sodium bicarbonate gargle.The observation group was treated with low energy laser irradiation based on the control group.Comparing the curative effect,the area of erosion,the degree of pain[visual analog scale(VAS score)]and the levels of secreted immunoglobulin A(IgA)and albumin(ALB)in saliva before treatment,1 month after treatment.Results The total effective rate of the observation group was 92.50%,significantly higher than that of the control group 75.00%(P<0.05);after 1 month of treatment,the erosion area and VAS score of the observation group were significantly less than those of the control group(P<0.05);after 1 month of treatment,the saliva IgA level in the observation group was significantly higher than that in the control group,and the ALB level was significantly lower than that in the control group(P<0.05).Conclusion Lowenergy laser irradiation combined with triamcinolone ointment topical rubbing and sodium bicarbonate gargle is effective in treating gingival erosive LP,which can reduce the area of erosion,reduce pain,and improve the level of IgA and ALB in saliva.
