摘  要:目的探讨产前超声检查在胎儿先天性心脏病诊断中的应用价值。方法对在梧州市妇幼保健院接受产前超声检查的4889例孕妇,常规检查之后对胎儿的心脏进行扫查,获得四腔心切面、左/右室流出道切面、三血管气管切面等,扫查各切面时适当放大图像并缩小对扇角的宽度,以详细观察胎儿的心血管形态.结构情况,包括心脏位置、形态、大小、房室瓣膜以及各大血管之间的关系等有无异常。结果产前经超声筛查出胎儿先天性心脏畸形53例,漏诊1例迷走左肺动脉、1例三尖瓣发育不良,发病率为1.12%(55/4889),产前超声筛查胎儿心脏畸形准确率为9636%,漏诊率为3.64%。结论产前超声对筛查胎儿先天性心脏畸形具有操作简便、安全无创、准确率高等优点,值得临床推广。Objective To explore the application value of prenatal ultrasound in the diagnosis of fetal congenital heart disease. Methods in our hospital for prenatal ultrasound examination of 4889 cases of pregnant women,after the routine inspection to scan the fetal cardiac four-chamber view, left/right ventricular outflow tract aspect,three-vessel-trachea view,etc.,to enlarge images appropriately when scanning each aspect and narrow width of fan Angle,in order to detailed observations of the fetal cardiovascular morphological structure,including the location,shape,size,atrioventricular valve,and the relationship between the presence of abnormal blood vessels. Results There were 53 cases of congenital heart malformation detected by prenatal ultrasound screening,1 case of left vagus pulmonary artery and 1 case of tricuspid valve dysplasia. The incidence rate was 1.12%(55/4889),the accuracy rate of prenatal ultrasound screening for fetal heart malformation was 96.36%,and the rate of missed diagnosis was 3.64%.Conclusion Prenatal ultrasound has the advantages of simple operation,safety,noninvasive and high accuracy in screening fetal congenital heart malformations,which is worthy of clinical application and popularization.
