同步放化疗与序贯放化疗法治疗局部晚期非小细胞肺癌的近期疗效和不良反应比较

摘　　要：目的比较同步放化疗与序贯放化疗法治疗局部晚期非小细胞肺癌的近期疗效和不良反应。方法选取2017年6月至2018年12月本院收治的54例局部晚期非小细胞肺癌患者,根据治疗方式的不同分为同步组和序贯组,每组27例。两组均以吉西他滨和顺铂为化疗药物。同步组给予同步胸部放疗和化疗,序贯组先接受3~4周化疗治疗,再胸部放疗2~3周期,放疗结束后再化疗1~3周期。比较两组近期疗效和不良反应发生率。结果同步组的肿瘤缓解率、肿瘤控制率、无进展生存率、总生存率均高于序贯组(P<0.05),其肿瘤远处转移率低于序贯组(P<0.05),且Ⅲ~Ⅳ级骨髓抑制、食管炎、胃肠道反应发生率高于序贯组(P<0.05)。结论同步放化疗与序贯放化疗法均能有效治疗局部晚期非小细胞肺癌,同步放化疗的近远期疗效更为理想,但不良反应发生率更高,临床治疗应根据患者情况合理用药。Objective To compare the short-term efficacy and adverse reactions of synchronous radiotherapy and sequential radiotherapy in the treatment of locally advanced non-small cell lung cancer.Methods 54 patients with locally advanced non-small cell lung cancer admitted to our hospital from June 2017 to December 2018 were divided into synchronous group and sequential group according to different treatment methods,with 27 cases in each group.Gemcitabine and cisplatin were used as chemotherapy drugs in both groups.The patients in the sequential group received chemotherapy for 3-4 weeks,followed by radiotherapy for 2-3 cycles,and then chemotherapy for 1-3 cycles after radiotherapy.Compared the short-term effect and adverse reactions between the two groups.Results The tumor response rate,tumor group synchronization control,progression-free survival and overall survival were higher than sequential group(P<0.05),the tumor distant metastasis rates lower than sequential group(P<0.05),and the synchronousⅢ-Ⅳmagnitude bone marrow inhibition,esophagitis,gastrointestinal tract reaction probability was higher than sequential group(P<0.05).Conclusion Both Synchronous radiotherapy and sequential radiochemotherapy can effectively treat locally advanced non-small cell lung cancer.The short-term and long-term curative effect of synchronous radiotherapy is more ideal,but the incidence of adverse reactions is higher,the clinical treatment should be based on the patient's condition.
