摘 要:目的分析早期肠外、肠内阶段性微生态免疫营养支持与单纯肠内营养对急性重症胰腺炎的改善效果。方法选取2017年10月至2019年9月收治的83例急性重症胰腺炎患者资料,根据患者意愿将其分为研究组(n=41)和对照组(n=42);对照组实施单纯肠内营养支持,研究组实施早期肠外、肠内阶段性微生态免疫营养支持;对比两组患者接受不同营养支持下肠道黏膜功能、营养状态及免疫功能改善效果。结果研究组内毒素、二氧化酶分别为(3.28±1.11)EU/ml、(2.99±0.82)ng/L,均低于对照组(4.79±1.29)EU/ml、(4.03±1.03)ng/L,差异均有统计学意义(均P<0.05);研究组总蛋白、白蛋白、前白蛋白、转铁蛋白分别为(67.57±11.87)g/L、(41.65±6.16)g/L、(296.29±30.43)mg/L、(2.95±0.65)g/L,均高于对照组(61.28±12.43)g/L、(37.91±7.15)g/L、(272.67±32.09)mg/L、(2.31±0.73)g/L,差异均有统计学意义(均P<0.05);研究组CD3+、CD4+、CD4+/CD8+指标分别为(70.49±4.37)%、(44.21±6.15)%、(1.89±0.32),均高于对照组(66.86±4.76)%、(40.47±5.34)%、(1.57±0.33),差异均有统计学意义(均P<0.05);两组患者CD8+指标比较,差异无统计学意义(P>0.05)。结论急性重症胰腺炎患者应用早期肠外、肠内阶段性微生态免疫营养支持,可有效保护肠道黏膜,提升免疫功能和营养状态,促进近远期预后效果,具有临床广泛推广和应用价值。Objective To analyze the effects of early parenteral and intestinal microecological immune nutrition support and only enteral nutrition on improvement of acute severe pancreatitis.Methods Eighty-three patients with acute severe pancreatitis treated at our hospital from October,2017 to September,2019 were selected,and were divided into a study group(n=41)and a control group(n=42)according to their will.The control group carried out enteral nutrition support,and the study group early parenteral and intestinal stage microecological immune nutrition support.The intestinal mucosal function,the nutritional status,and the immune function of the two groups were compared.Results The levels of endotoxin and dioxygenase were lower and the levels of total protein,albumin,prealbumin,transferrin,CD3+,CD4+,and CD4+/CD8+were higher in the study group than in the control group[(3.28±1.11)EU/ml vs.(4.79±1.29)EU/ml,(2.99±0.82)ng/L vs.(4.03±1.03)ng/L,(67.57±11.87)g/L vs.(61.28±12.43)g/L,(41.65±6.16)g/L vs.(37.91±7.15)g/L,(296.29±30.43)mg/L vs.(272.67±32.09)mg/L,(2.95±0.65)g/L vs.(2.31±0.73)g/L,(70.49±4.37)%vs.(66.86±4.76)%,(44.21±6.15)%vs.(40.47±5.34)%,and(1.89±0.32)vs.(1.57±0.33);all P<0.05].There was no statistical difference in CD8+between the two groups.Conclusion The application of early parenteral and intestinal microecological immune nutrition support in patients with severe acute pancreatitis can effectively protect their intestinal mucosa,improve their immune function,nutritional status,and short-and long-term prognosis,so it has the value of extensive clinical promotion and application.
