肿瘤坏死因子-α瘤内注射联合化疗治疗晚期非小细胞肺癌疗效观察

摘　　要：目的探讨肿瘤坏死因子-α(TNF-α)瘤内注射联合化疗治疗晚期非小细胞肺癌的疗效和安全性。方法选择86例经病理学或细胞学证实且不可手术切除的晚期非小细胞肺癌患者,随机均分为对照组和治疗组,每组43例。对照组单纯采用化疗:多西他赛75 mg/m^2,d 1,静脉滴注;顺铂75 mg/m^2,d 1,静脉滴注;21 d为1个周期。治疗组在对照组相同化疗的基础上,于化疗第2天给予肺部肿瘤重组改构TNF-α瘤内注射,根据肿瘤直径每1 cm给予100万IU TNF-α,共治疗3个周期,3个周期后行计算机断层扫描检查测量病灶,评估疗效。结果治疗组疾病控制率为69.8%,高于对照组的39.5%,差异有统计学意义(χ^2=7.929,P=0.005)。治疗组和对照组肾功能异常、神经毒性、恶心呕吐、肝功能异常、白细胞减少发生率比较差异均无统计学意义(χ^2=0.000,P=1.000;χ^2=0.000,P=1.000;χ^2=0.244,P=0.621;χ^2=0.059,P=0.808;χ^2=0.205,P=0.651)。结论TNF-α瘤内注射联合化疗可提高晚期非小细胞肺癌的疗效,且安全性高。Objective To investigate the efficacy and safety of tumor necrosis factor-α(TNF-α)intratumoral injection combined with chemotherapy in the treatment of advanced non-small cell lung cancer.Methods Eighty-six patients with advanced non-small cell lung cancer confirmed by pathology or cytology were randomly divided into the control group and the treatment group,43 patients in each group.Chemotherapy alone was used in the control group:docetaxel 75 mg/m^2,d 1,intravenous drip;cisplatin 75 mg/m 2,d 1,intravenous drip;21 days as a cycle.TNF-αintratumoral injection combined with chemotherapy were used in the treatment group:at the basis of chemotherapy,TNF-α was injected into lung tumor on the second day of chemotherapy.According to the diameter of the tumor,1 million IU TNF-αwas given every 1 cm.After 3 cycles,computed tomography was performed to measure the focus and evaluate the therapeutic effect.Results The disease control rate of in the treatment group(69.8%)was significantly higher than that of in the control group(39.5%;χ^2=7.929,P=0.005).There were no significant statistical differences in the renal dysfunction,neurotoxicity,nausea and vomiting,liver dysfunction,leukopenia between the two groups(χ^2=0.000,P=1.000;χ^2=0.000,P=1.000;χ^2=0.244,P=0.621;χ^2=0.059,P=0.808;χ^2=0.205,P=0.651).Conclusion TNF-αintratumoral injection combined with chemotherapy can improve the efficacy in the anti-tumor treatment of advanced non-small cell lung cancer,and the safety is good.
