放射性125Ⅰ粒子组织间植入在颌面部腺源性恶性肿瘤治疗中的应用

摘　　要：目的探讨放射性碘125(125Ⅰ)粒子组织间植入对颌面部腺源性恶性肿瘤的作用。方法选取97例颌面部腺源性恶性肿瘤患者进行回顾性分析,患者分为观察组和对照组,经倾向性评分匹配,每组各纳入31例。观察组患者术后行放射性125Ⅰ粒子组织间植入近距离放疗,对照组患者术后行常规外照射放疗。比较观察2组患者的治疗效果和不良反应。结果观察组3 a局部控制率为83.87%,高于对照组的58.06%,差异有统计学意义(χ^2=5.010,P=0.025)。观察组口腔黏膜炎、口干症、皮炎、颌骨坏死等放疗不良反应发生率分别为9.68%、3.23%、0.00%、0.00%,均明显低于对照组的74.19%、67.74%、87.10%、19.35%,差异均有统计学意义(χ^2=26.496,P<0.001;χ^2=28.182,P<0.001;χ^2=47.829,P<0.001;χ^2=2.736,P=0.032)。结论放射性125I粒子组织间植入可有效控制颌面部恶性肿瘤术后进展,且安全性较高。Objective To evaluate the application of implanting radioactive 125Ⅰ particle in the treatment of maxillofacial region adenogenic malignant tumors.Methods Ninety-seven postoperative patients with maxillofacial region adenogenic malignant tumors were selected for retrospective analysis. The patients were divided into two groups, 31 patients in the observation group received brachytherapy with the implanting radioactive 125Ⅰ particle after surgery, 31 patients in the control group received traditional external radiotherapy after surgery. The clinical efficacy and toxicities were compared between the two group. Results The 3-year local control rate was 83.87% in the observation group, and was 58.06% in the control group, the difference was statistically significant(χ^2=5.010,P=0.025). The incidences of oral mucositis, xerostomia, dermatitis and jaw necrosis in the observation group were respectively 9.68%, 3.23%, 0.00%, 0.00%, and were respectively 74.19%, 67.74%, 87.10%, 19.35%, the differences were statistically significant(χ^2=26.496,P<0.001;χ^2=28.182,P<0.001;χ^2=47.829,P<0.001;χ^2=2.736,P=0.032).Conclusion Implanting radioactive 125I particle can effectively control the maxillofacial region adenogenic malignant tumors, and the safety is high.
