老年直肠癌根治术患者术后吻合口瘘危险因素分析

摘　　要：目的探讨老年直肠癌根治术患者术后吻合口瘘的危险因素和预防、处理措施。方法回顾性分析2015年1月—2020年1月在本院行直肠癌根治术的86例老年患者的临床资料,其中出现吻合口瘘的患者归为病例组(n=14),余患者归为对照组(n=72)。分析两组患者临床资料的差异性,探讨影响吻合口瘘的危险因素及处理对策。结果吻合口瘘发生率为16.28%;患者年龄、术前血红蛋白、肿瘤位置是导致吻合口瘘的危险因素,且男性患者,年龄、BMI、肿瘤大小与吻合口瘘的发生呈正相关,与术前血红蛋白、肿瘤距肛缘距离呈负相关。结论高龄、术前血红蛋白<100 g/L、肿瘤距肛缘距离<7 cm是老年直肠癌患者术后吻合口瘘的危险因素。Objective To explore the risk factors,prevention and treatment of anastomotic leakage after radical resection of rectal cancer in elderly patients.Methods The clinical data of 86 cases of elderly patients undergoing radical rectal cancer surgery in our hospital from January 2015 to January 2020 were retrospectively analyzed.Among them,patients with anastomotic leakage were classified as the case group(n=14),and the remaining patients were classified as controls Group(n=72).Analyzing the differences in the clinical data of the two groups of patients,to explore the risk factors and treatment strategies that affect anastomotic leakage.Results Anastomotic leakage occurred in 14 of 86 cases of patients with rectal cancer after radical resection,with an incidence of 16.28%.Age of patients,preoperative hemoglobin and tumor location were the risk factors of anastomotic fistula.In male patients,age,BMI and tumor size were positively correlated with the occurrence of anastomotic leakage,and negatively correlated with preoperative hemoglobin and the distance between tumor and anal margin.Conclusions The advanced age,preoperative hemoglobin less than 100g/L,distance between tumor and anal margin less than 7cm were risk factors for anastomotic leakage in elderly patients with rectal cancer.
