下颌骨骨折坚强内固定术后感染治疗的经验和教训

摘　　要：目的:通过下颌骨骨折坚强内固定术后感染患者的治疗,分析感染可能发生的原因和治疗措施,为临床工作提供经验教训。方法:对25例下颌骨骨折行坚强内固定术后感染患者,通过细菌培养、彻底清创、全身抗炎和局部换药进行治疗,并对感染发生的原因进行分析。结果:主要致病菌为金黄色葡萄球菌、肺炎克雷伯杆菌、凝固酶阴性葡萄球菌、大肠埃希菌,死腔、异物和口腔卫生差是感染发生的可能原因。治疗后感染得到良好控制。结论:死腔、异物和局部卫生条件差会增加感染的几率。Objective:To analyze the possible causes and treatment measures of infection after rigid internal fixation of mandibular fracture for providing experience and lessons in clinie.Methods:Twenty-five cases of mandibular fractures appeared infection after rigid internal fixation.All patients were treated by bacterial culture,thoroughly debridement,systemie anti-inflammatory and local change dressing.Meanwhile,the causes of infection were analyzed.Results:The main pathogenic bacteria including Staphy lococcus aureus,Klebsiella pneumonia,Coagulase negative staphylococcus and Escherichia coli.Dead space,foreign objects and poor 0ral hygiene might be the causes of the infection.The infection was controlled well in all cases.Conclusion:Dead space,foreign bodies and poor local condition can increase the probability of infection.
