社区老年人社会隔离对生活质量的影响路径分析

摘　　要：目的基于抑郁和活动能力两个中介变量,构建多重中介模型,探讨社区老年人社会隔离对其生活质量的影响机制。方法便利抽样选取青岛市592名社区老年人,采用一般资料问卷、Lubben社会网络量表-6、老年抑郁量表、日常生活活动能力量表及简明健康状况量表对其进行调查。结果社会隔离得分与生活质量评分呈正相关,与抑郁和活动能力评分呈负相关;抑郁和活动能力得分与生活质量评分呈负相关(均P<0.01)。路径分析显示,社会隔离可直接影响老年人生活质量,还可通过抑郁、活动能力的部分中介作用间接对老年人生活质量产生影响;抑郁和活动能力的中介效应分别占总效应的27.82%和24.97%。结论社区护理作为促进和维护老年健康的重要力量,在护理实践中应重视社会隔离对老年人生活质量的影响,并基于作用路径实施干预管理,以提高老年人生活质量,促进健康老龄化。Objective To construct a multiple mediation model and explore the influencing mechanism of social isolation on quality of life among community-dwelling elderly based on two mediating variables of depression and activity ability.Methods Convenient sampling was used to select 592 community-dwelling elderly in Qingdao City for investigation,using the general information questionnaire,Lubben Social Network Scale-6,Geriatric Depression Scale,Activities of Daily Living Scale and SF-36 Brief Health Scale.Results The score of social isolation was positively associated with quality of life,and negatively correlated with depression and activity ability;the scores of depression and activity ability were negatively correlated with quality of life(all P<0.01).Path analysis showed that social isolation could directly affect the quality of life of the elderly,and exert an indirect influence on their quality of life through the partial mediating effects of depression and activity capacity,which accounted for 27.82%and 24.97%of the total effect,respectively.Conclusions As an important role to promote and maintain the health of the elderly,community nursing practice should put emphasis on the impact of social isolation on their quality of life.Community should implement interventions based on acting paths,so as to improve the quality of life of community-dwelling elderly and promote healthy aging.
