摘　　要：目的探讨脉管瘤栓在Ⅱ期结直肠癌中的临床意义。方法系统性搜集行手术治疗的Ⅱ期结直肠癌患者临床资料和随访资料,分析患者脉管瘤栓情况与临床特征的关系和其对预后的影响。结果共纳入305例Ⅱ期结直肠癌病例,其中伴有脉管瘤栓者67例(22.0%)。脉管瘤栓情况在年龄、分化程度、pT分期和神经侵犯情况方面比较,差异均具有统计学意义(P<0.05)。多因素分析发现,脉管瘤栓及神经侵犯均为影响Ⅱ期结直肠癌无瘤生存及总生存的独立危险因素(均P<0.05)。结论脉管瘤栓为Ⅱ期结直肠癌预后的独立危险因素。Objective To explore the clinical significance of vascular invasion in stageⅡcolorectal cancer(CRC).Methods The data of stageⅡCRC patients undergoing surgery were systemically recorded and analyzed,and the association between the presence of vascular invasion,clinical characteristics and prognosis was analyzed.Results Totally 305 patients with stageⅡCRC were included in this study,among whom 67(22.0%)patients had vascular invasion.There were statistically significant differences in the status of vascular invasion in terms of age,degree of differentiation,pT stage,and nerve invasion(all P<0.05).Multivariate analysis showed that vascular invasion and nerve invasion were independent risk factors affecting disease-free survival and overall survival of stageⅡCRC(both P<0.05).Conclusion Vascular invasion is an independent risk factor of stageⅡCRC.
